0474192806 2015-02-25 Casing String Witnessed No Violations On the date of inspection I am on site to witness the cementing of the surface casing. I was informed by American Energy Utica consultant Billy Fritt that the Nomac 84 rig has drilled a 17.5 inch diameter hole to a total depth of 399 feet, spudded on 2-24--Tex Pumping, LLC pumped 630sk of Class H cement, 15.6ppg, 1.2 yield, 2% CaCl2, 0.2% X-Air, 0.25lbs/sk of Cello Flake. O-Tex pumped 55bbl of water to displace cement. Plug was bumped, floats held, and 20bbl of cement returned to surface. Weather conditions at the time of cementing were icy with light snow and temperatures in the upper teens. There was approximately one inch of ice on location and five inches of snow off location.

1179757087 2016-07-26 Preliminary Restoration No Violations I arrived on location, where I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells have not been fracked, however the production equipment has been place on location. RBDMS indicated that the well is not in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 2 NGL Tanks 6 Non-fracked wells with night caps

1 Vapor recovery system 1 combustor 1 Low pressure separator

2 heater treaters

1 Line Heater The site passed preliminary restoration in accordance with ORC 1509.072 A.

1426167709 2015-03-12 Drill / Deepen / Reopen No Violations On the day of my inspection I found the crew drilling at a depth od 12300 feet. They are getting ready to make a bit trip. In walking the location, I did not find any issues that need address. The well should be TD by the end of the week.
1593458108 2015-03-04 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by American Energy Utica consultant Larry Hart that the Nomac 84 rig was at a depth of 6475 feet. O-Tex had cemented the intermediate string on 2-27-
1958519500 2018-01-10 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain six christmas tree wellheads. Production equipment includes three line heaters, six gas production units, six vertical sand seperators, twelve 400 barrel steel tanks, two compressors, 2 vapor recovery towers, a combustor, and pipeline equipment. This well had zero pressure on the surface gauge and 50 psi on the intermediate gauge. Production records are current through 2017. Sign needs changed to 34-067-2-1372-01-00. Site meets requirements of ORC 1509.072A for preliminary restoration.
