0445484862 2014-08-18 Drill / Deepen / Reopen No Violations On the date of inspection I accompanied ODNR Inspector Mark Moreland to check the status of the conductor hole being drilled and when it would be cemented. Refer to Mark Morelands report for further details.
0667951770 2014-10-15 Casing String Witnessed No Violations The rig ran 572 of 20 casing and the associated float equipment. O-Tex pumping Service cemented it with 580 sacks of Class H cement. Good cement was returned to the surface. See the casing ticket for the details.
0772283358 2014-10-30 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling status of Patterson Drilling Rig # 256 I found that they are waiting on cement and that they will trip in the hole soon to drill out the shoe at 6865 ft. Next they need to complete a Cement Bond Log and a Formation Integrity Test. The cement bond log is to see how much casing is cemented in place. There may be casing which does not have cement behind it. The Formation test is to see if the casing is deep enough to seal off the fluid loss zones.
0857837718 2016-01-05 Preliminary Restoration No Violations On the day of my inspection accompanied by Inspector Doug Bennett, I witness the well head is equipped with a production tree with valves in the open position. The production equipment included a sand trap and a GP unit. Also present were 3 stabilizer units, 2 line heaters, 2 compressor units, 3 NGL tanks, 2 combustors, 8- 500bbl condensate tanks and 4-500bbl brine tanks all within proper containment. This equipment is shared with the 7H, 3H, 1H, 2H, 4H, 6H, 8H and 10H. The site passed preliminary restoration in accordance with ORC 1509.072 A.
1408484749 2014-08-19 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling status of the conductor hole. The hole collapsed at 65 ft. and had to be filled in with ready mix grout cement to stabilize the well bore. They should be ready tonight to cement conductor in place.
1440665950 2016-01-05 Production Wells No Violations On the day of my inspection I accompanied ODNR Inspector Joe Zollars please refer to his report for details.
1455950482 2016-03-01 Preliminary Restoration No Violations This well is in production and meets the requirements for the preliminary restoration. This well is one of 9 wells in operation on this pad. Each well has to have proper identification. To see a list of the equipment on this pad used for the production of these wells see permit # 1366
1758479741 2014-08-19 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 26 inch diameter casing string. Precision Drilling drilled a 30 inch diameter hole to 100 ft. and set 100 ft. of casing. O-Tex cemented with 190 sacks of class H cement at 15.6 lb. and a yield of 1.20 had 5 bbls of cement returns.
1996093860 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains nine properly labeled producing wellheads utilizing a plunger lift system. Production equipment includes eight 500 barrel steel tanks in their own polymer lined steelcontainment, nine GPUs, nine sand seperators, three horizontal dehydrators, a compressor, and a vapor recovery unit. Surface annular pressure is 150 and intermediate annular pressure is 0. Production records are current in RBDMS.
