They requested the meeting to find out more details of the changes to the casing program which have been proposed by ODNR. I did advise the group that the final changes have not been made known to anyone at this time. I also advised that changes will continue for some time and if they wanted to meet with ODNR that they could do so. Later in the day I advised Deputy Chief Dave Claus and Bob Worstall of the request to meet with A.E.U.
0656803056 2016-03-01 Preliminary Restoration No Violations This well is in production and meets the requirements for the preliminary restoration. This well is one of 9 wells in operation on this pad and has proper identification. To see a list of the production equipment see permit # 1366
1187467181 2016-01-05 Preliminary Restoration No Violations On the day of my inspection accompanied by Inspector Doug Bennett, I witness the well head is equipped with a production tree with valves in the open position. The production equipment included a sand trap and a GP unit. Also present were 3 stabilizer units, 2 line heaters, 2 compressor units, 3 NGL tanks, 2 combustors, 8- 500bbl condensate tanks and 4-500bbl brine tanks all within proper containment. This equipment is shared with the 5H, 3H, 1H, 2H, 4H, 6H, 8H and 10H. The site passed preliminary restoration in accordance with ORC 1509.072 A.
1254475309 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains nine properly labeled producing wellheads utilizing a plunger lift system. Production equipment includes eight 500 barrel steel tanks in their own polymer lined steel containment, nine GPUs, nine sand seperators, three horizontal dehydrators, a compressor, and a vapor recovery unit. Surface annular pressure is 0 and intermediate annular pressure is 0. Production records are current in RBDMS.
1264206094 2014-08-21 Casing String Witnessed No Violations I am on site tonight to witness the running and the cementing of the 26 inch diameter casing string. Precision Drilling drilled a 30 inch diameter hole to 100 ft. and set 100 ft. of casing. O-Tex Cementing Service cemented with 190 sacks of class H cement at 15.6 and a yield of 1.20 had only 3 bbls of return.

1410351382 2014-09-06 Casing String Witnessed No Violations The rig ran 20 casing and the asociated float equipment to 553. O-Tex cementing cemented the casing with 580 sacks of class A cement. See the casing ticket for the details.
1410367757 2014-09-10 Casing String Witnessed No Violations On the day of my inspection I found that AEU was not ready to cement casing. Patterson Drilling rig 256 had drilled a 17.5 hole to a depth of 1990. They were running new, centralized 13.375 casing. They were having problems getting the last two joint of casing in the hole. They were trying to wash the joints in.
1410867289 2014-09-15 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling status of Patterson Drilling Rig # 256 and found that they are at 6700 ft. on vertical section of the well. They will set a deep intermediate casing which they plan not to run a DV Tool. That means if the formation integrity does not support the weight of the cement they will not be able to circulate cement to the upper casing string to tie them together.
1411388885 2014-09-22 Drill / Deepen / Reopen No Violations Patterson Drilling Rig # 256 drilled to 8082 ft. and was off the proposed drilling track. They plugged back to correct the drill track. Today during my inspection I found that they are waiting on cement for 24 hours and will begin drilling at 6025 ft. to begin rebuilding the curve.
1412096434 2014-09-30 Drill / Deepen / Reopen No Violations I am on site today to check on the Patterson Drilling Rig # 256 and found them drilling at 10,976 ft. drilling a 8.5 inch diameter hole with oil base drilling mud. Move to the # 5 -H well after this is complete.
1498259209 2014-09-17 Casing String Witnessed No Violations Patterson Drilling Rig # 256 drilled a 12-1/4 inch diameter hole to 7110 ft. and set 7100 ft. of 9-5/8 inch diameter 40 lb casing. O-Tex Cementing Service cemented with 1090 sacks of lead cement at 12 lb and yield of 2.15 and a tail cement of 220 sacks at 15.3 and a yield of 1.27 did not get cement returns and advised Cecil the Company Man for AEU to complete a bond log before drilling out from under cement to determine the top of the cement. This job was not a 2 stage cement job and they did not run a DV Tool.
