1140837523 2014-07-30 Casing String Witnessed No Violations Onsite with Inspector Mark Moreland to witness cementing of the 9 5/8 intermediate casing. Patterson 261 drilled a 12.25x2111 open hole and set 2110 of 9 5/8 40#/ft casing. Otex was onsite with 770 sacks of class H (15.8ppg; 1.21 yield) to cement the casing with 40% excess. 30 bbl of cement was returned to surface. Displacement calculated to be 152bbl.
1404315860 2014-07-02 Casing String Witnessed No Violations On the date of inspection, I accompanied ODNR Inspector Mark Moreland to witness the running and cementing of the 20 conductor casing. Express Drilling drilled a 26 hole to a depth of 120 and set 120 of 94# casing. Express Cementing Service cemented with 230sks Class A at 15.6# and yield of 1.18. Company man present on site is Brett Farnsworth. Express Energy Services established circulation in hole before cementing and received 10bbl of cement back to surface. Cement stayed to surface while I was on site.
1406491988 2014-07-27 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 13-3/8 inch diameter casing string. Patterson Drilling Rig # 261 drilled a 17.5 inch diameter hole to 585 ft and set 574 ft of casing. O-Tex Cementing Service cemented with 527 sacks of class A cement at 15.6 lbs and a yield of 1.20 had an intense storm during the casing installation and the job was shut down for 1 hour.
1406709411 2014-07-30 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 9-5/8 inch diameter casing string. Patterson Drilling Rig # 261 drilled a 12-1/4 inch diameter hole to 2111 ft. and set 2010 ft. of 40 lb. casing. O-Tex Cementing Service cemented with 770 sacks of class H cement at 15.8 lb. and a yield of 1.21 and had 30 bbls of good cement to surface.
1596546261 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 4 compressors 2 NGL tanks The site passed preliminary restoration in accordance with ORC 1509.072 A.
