0010839885 2014-10-24 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by Chesapeake consultant Joe Herbert that a Side Track Plug was set the previous night. Joe informed me that the top of the tools was at a depth of 5676 feet. A Side Track Plug was set from approximately 5668 feet to 4892 feet with 10% excess cement. I will be informed of the top of cement when it is tagged and their Kick Off Point from the plug. I informed him I wanted there to be a minimum of 500 feet of the plug to remain in the hole after they drill out part of the top. The Side Track Plug was set by O-Tex Pumping, LLC. O-Tex pumped 350sk of Class H cement, 17.5ppg, 0.94 yield, 0.4% C-35, and 0.4% C-41P in 8.75 inch open hole. O-Tex pumped 13bbl of spacer behind the plug at 14.0ppg and 70bbl of Oil Based Mud at 10.5ppg. They shut down when pressure climbed. Cement and spacer was set through 5 inch 19.5lbs/ft drill pipe. The total equipment left in hole was 862.31 feet, that includes; 15 joints of Heavy Weight Drill Pipe @ 453.69ft, X-O Sub @ 2.66ft, 3 --O Sub @ 1.51ft, Non-Magnetic Drill Collar @ 31.06ft, Antenna Sub @ 3.32ft, Indexing Sub @ 3.14ft, Non-Magnetic Drill Collar @ 31.08ft, Non-

0475552734 2014-11-03 Drill / Deepen / Reopen No Violations On the date of inspection I was on site to check the status of the Side Track drilling that had taken place. I was informed by Chesapeake consultant Joe Hebert that the Patterson 261 rig was able to Kick Off from the plug at 5100 feet. The bottom of the plug was set at 5676 feet. The rig was currently at a depth of 7462 feet and beginning to build the curve.

At the time of my inspection, Franks Casing Crew was currently running 5.5 inch casing. It would be several more hours before O-Tex Pumping LLC would be able to pump the cement job. I left the site to help cover a job for another inspector.

1413393597 2014-10-15 Casing String Witnessed No Violations On the date of inspection I am on site to witness the Intermediate Casing String due to missing Surface String for a holiday. I was informed by Chesapeake consultant Rodney Bean that the Patterson 261 rig had drilled a 12.25 inch diameter hole to a total depth of 2214 feet. Wyoming Casing Crew set 9.625 inch 40.00lbs/ft casing to a depth of 2203 feet. O-Tex Pumping, LLC pumped 810sk of Class H cement, 15.8ppg, 1.21 yield, 3% Gypsum, 2% CaCl, 0.8% MC-500, 0.5% C-45, 0.4% C-41P. O-Tex displaced cement with 157bbl of water. 37bbl of cement returned to surface and stayed at surface while I was on site.

1531349156 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 4 compressors 2 NGL tanks The site passed preliminary restoration in accordance with ORC 1509.072 A.

1722216988 2014-10-21 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by Chesapeake consultant Rodney Bean that the Patterson 261 was currently stuck down hole. The rig was drilling an 8.75 inch diameter hole at a depth of 6630 feet (6266 feet Total Vertical Depth) when the drill pipe became stuck at a depth of 5870 feet. According to Rodney, the drill pipe was stuck in place at the transition out of the Lockport formation. Blue Dot Pipe Retrieval will be working to release the pipe using jarring tools.
1970805271 2014-10-27 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by Chesapeake consultant Michael Smith that the Patterson 261 rig is currently Timed Drilling at a depth of 5012 feet. I was informed by Joe Hebert and Michael Smith over the weekend that the top of cement for the first Side Track Plug was tagged at 4967 feet. At this depth the cement was too soft to Side Track from. They drilled to a depth of 5387 feet (approximately 289 feet frombottom of cement) and the cement was still to soft to Kick Off from. O-Tex Pumping, LLC set a second Side Track Plug at 5387 feet with 300sk of Class H cement, 0.4% C-35, 0.4% C-41P, 17.5ppg, 0.94 yield. O-Tex pumped 13bbl of 14.0ppg spacer, 60bbl Oil Based Mud at 10.5ppg, and two wiper balls. The rig tagged top of cement at 4886 feet and began to Side Track by Timed Drilling at 4891 feet.
