0514215977 2016-02-23 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

11 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 5 compressors 2 NGL tanks The site passed preliminary restoration in accordance with ORC 1509.072 A.

1039051643 2014-12-01 Not Drilled No Violations I inspected this well on 12/1/2014. Patterson Rig # 329 is on location and performing maintaince on the top drive. The expected spud date is 12/2/2014. A walk around inspection revealed no violations at this time.
1418648914 2014-12-04 Casing String Witnessed No Violations I was on the R Alan # 1 - H location to witness the cementing of the 13.375 surface casing. Patterson Drilling, Rig # 329, drilled a 17.5 borehole from 145 KB to 794 KB and ran 17 joints of 13.375, 54.5 lbs. / foot casing to 762. Schlumberger pumped 25 barrels of gelled spacer and 10 barrels of water ahead of the cement slurry. The cement slurry was comprised of 780 sacks of Class A cement. Casing hardware included 7 bowspring centralizers, 13.375 float shoe and 13.375 float collar. The displacement plug landed at 316 psi and was bumped to 900 psi. After 5-minute shut in the pressure was 881 psi. Upon releasing the pressure back 1/2 barrel flowed back and stopped. All floats held. I witnessed approximately 55 barrels of cement slurry returned to surface. The mix water temperature was 55 and pH was 7. No violations were identified during this inspection.
1911347749 2014-10-11 Casing String Witnessed No Violations Rocky Mountain Drilling drilled 120 of 24 hole and set 120 of 20 conductor. Univeral Well Services cemented it with 245 sacks of Class A cement. Good cement returned to the surface. See the casing ticket for the details.
1920242539 2014-12-06 Casing String Witnessed No Violations I was on the R Alan # 1 - H location to witness the cementing of the 9.625 intermediate casing. Patterson Drlg., Rig 329, drilled a 12.25 borehole from 794 KB to 1891 KB and ran 43 joints of 9.625, 40 lbs. / foot casing to 1891 KB. Schlumberger pumped 25 barrels of gelled spacer and 10 barrels of water ahead of the cement slurry. The cement slurry was comprised of 705 sacks of Class A cement. The casing hardware included 15 bowspring centralizers, a 9.625 float shoe and 9.625 float collar. The displacement plug was landed at 734 psi and subsequently bumped to 1420 psi and held for 5 minutes. Green cement test was within tolerances. Upon releasing the pressure 3/4 barrel flowed back and stopped. All floats held. I witnessed 31 barrels returned to surface weighing 15.1 pounds per gallon. No violations were identified during this inspection.
