0354103092 2014-07-29 Drill / Deepen / Reopen No Violations Patterson Drilling Rig # 256 is at 4923 ft. drilling a 12-1/4 inch diameter hole and is down for repairs to the draw works motor.
0355726320 2014-08-02 Casing String Witnessed No Violations On location to witness the 2nd stage of a 2 stage cement job of the 9-5/8 casing. The DV tool at 3387 was open and the rig had been circulating for several hours. O-Tex Cementing pumped 298 bbls of Low Densinty cement and displaced it to the surface with 257 bbls of KCl water. 87 bbls of good cement were returned to the surface. The DV tool closed as designed when the plug reached bottom. The plug was bumped to 1500 psig over landing pressure and held for 15 minutes then released. It was bumped up again and released after 2 minutes. 1.5 bbls of KCl water was returned each time and the DV tool held. See the casing ticket for the details.
0517592786 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains nine properly labeled producing wellheads utilizing a plunger lift system. Production equipment includes eight 500 barrel steel tanks in their own polymer lined steel containment, nine GPUs, nine sand seperators, three horizontal dehydrators, a compressor, and a vapor recovery unit. Surface annular pressure is 250 and intermediate annular pressure is 500. Production records are current in RBDMS.
0915849658 2014-08-11 Drill / Deepen / Reopen No Violations Patterson Drilling Rig # 256 is moving to the 10 - H well today. They should be drilling tonight and ready to set surface Tuesday Evening.
1264115330 2016-01-05 Preliminary Restoration No Violations On the day of my inspection accompanied by Inspector Doug Bennett, I witness the well head is equipped with a production tree with valves in the open position. The production equipment included a sand trap and a GP unit. Also present were 3 stabilizer units, 2 line heaters, 2 compressor units, 3 NGL tanks, 2 combustors, 8- 500bbl condensate tanks and 4-500bbl brine tanks all within proper containment. This equipment is shared with the 7H, 5H, 3H, 1H, 2H, 4H, 6H and 10H. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1406387276 2014-07-26 Casing String Witnessed No Violations On the day of my inspection I found that Patterson Drilling rig 256 had drilled a 17.5 hole to a depth of 1035. They ran new 54.5 lb/ft 13.375 surface casing equipped with 6 centralizers. O-Tex Pumping cemented the casing to surface with 900 sacks of cement. Good circulation was maintained throughout the job with approximately 50 barrels of cement returns.
1406991203 2014-08-02 Casing String Witnessed No Violations On the date of inspection I was informed by American Energy Utica Company Man Chris Burt that the Patterson 256 rig had drilled with a 12.25 inch diameter bit to a Total Depth of 7205 feet with a floating shoe at a depth of 7195 feet. Wyoming Casing Crew set 9.625 inch 40 lbs./ft. casing to a depth of 7195 feet. O-Tex Pumping established circulation and pumped a lead cement of 1,420 sacks of Low Density 12.0ppg 2.15 yield. O-Tex pumped a tail cement of Class H 220 sacks with a weight of 15.2ppg and a yield of 1.27. Cement did not return to surface. The Patterson 256 rig opened the diversion tools at a depth of 3800 feet and began circulation.
1408624581 2014-08-11 Drill / Deepen / Reopen No Violations Patterson Drilling Rig # 256 is moving today to the # 10 well and should begin drilling Tuesday.
1434460760 2015-06-15 Production Wells No Violations I am on site today to check on the status of the well. I was informed by Stan Cherry that a casing collapse had occurred on the 5-1/2 inch production casing at 67 inches. Stan said that it was found when they began to work on the well and the service rig could not get a wire line below that depth. It was later discovered that the 9-5/8 inch diameter casing had collapsed at the same depth. The well has not been fracked and does not have pressure on it at this time. AEU has been advised that they are to submit a written plan for remediation to Deputy Chief Dave Claus.
1776386513 2016-03-01 Preliminary Restoration No Violations This well is in production and meets the requirements for the preliminary restoration. This well is one of 9 wells in operation on this pad and has proper identification. To see a list of the production equipment see permit # 1366
1994734705 2015-08-14 Production Wells No Violations Ascent Resources contacted me to advise that they would be moving equipment on site to begin fracking. They are still rigging up equipment and should begin in a day or two. They have a 10k snubbing unit on site for the completion work.
