0493179028 2017-09-22 Preliminary Restoration No Violations On the day of my inspection I found the Cadiz C pad to contain five properly labeled wellheads. Wells are equipped with chritmas tree wellheads. Gauges show no surface or intermediate casing pressures. Production equipment on the pad includes five gas production units, one horizontal seperator, one compressor, four 400 barrel steel condensate tanks, and a flare. Tanks are in their own steel containment. Production records current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A.

1416410705 2014-11-19 Drill / Deepen / Reopen No Violations On the date of inspection I met with Hess consultants Kevin Toothman and Mike Roberts. They informed me that during the previous day the HAD 245 rig had drilled a 22 inch diameter hole to a depth of 574 feet. They switched to a 17.5 inch diameter hole from 574 feet to the Top of Mine at 613 feet. The rig tagged the Bottom of Mine at 621 feet. As the rig drilled through the Top of Mine they ran into coal bed methane. They shut off the air and the methane was diverted to the flare stack. The flare has been burning since 15:45 11-18-14 through the time of my arrival at 10:00 11-19-14. Mike Roberts estimated the flare to be as high as 20-30 feet the previous night. The flare has diminished overnight to approximately 10-15 feet. Mike and Kevin plan to load the drill pipe and mine void with 1000-1500bbl of fresh water in an attempt to hold back the gas. I was forced to leave the site for another job after they had pumped 570bbl of fresh water through the drill pipe. The flare had diminished to approximately 1-2 feet. Kevin and Mike will follow up with ODNR Inspector Mark Moreland when the gas pressure dies down.
1416621911 2014-11-21 Casing String Witnessed No Violations On the day of my inspection I found HAD Drilling preparing to run 13 3/8 inch casing, this covering the mine string . The mine shaft was at 613 to 624 feet, and drilled to 668 feet. The cement baskets are set at the top being 566 feet and the other at 586 feet. Halliburton will doing the cementing. The casing would not go to bottom. They had tight hole conditions. The casing was removed from the wellbore and the hole was reamed.
1416783725 2014-11-23 Casing String Witnessed No Violations On the day of my inspection I found that HAD Drilling Had reamed the wellbore and reinstalled the casing the original Depth of 668 feet. The cement baskets were installed at 570 fee and at 590 feet 620 sacks of cement was pumped in the bottom. There was no circulation or returns to surface. 63 feet of 1 inch tubing was run on the outside of the 18 5/8 inch casing to grout the pipe to surface. They pumped 570 sacks with no returns to surface. They shut down and order more cement to attempt again to cement to surface.
1416930574 2014-11-24 Casing String Witnessed No Violations HAD Drilling Rig # 249 set 690 ft. Of 18-5/8 inch diameter casing c/w 620 sacks of class A cement on the bottom 50 ft. section below the Mine. Next they cemented the annulus with 1 inch tubing from the basket set at 542 ft. to surface . The basket did not deploy for the original job and a second job was done that did circulate cement to surface. 805 sacks total with 295 sacks used for the last job.
1437402994 2015-07-20 Drill / Deepen / Reopen No Violations Upon arrival on location I found the crew drilling ahead at 6840 feet. There are no issues at this time.
1927270320 2014-11-18 Drill / Deepen / Reopen No Violations I received a call from Company Man Kevin Toothman that HAD Rig # 249 drilled into a Mine at 600 ft. and that they blew out. Drilling has stopped and it is being diverted to the flare stack at this time. There is no uncontrolled fluid flow just a gas blow out. The plan now is to go back in and drill the 50 ft. section to land the Mine String casing after the gas blows down.
