0082733355 2016-12-07 Preliminary Restoration No Violations On the day of my inspection I witness the lease road was in good repair and the gates to the location were open due to service companies working on the well bore. Upon my arrival I witnessed that the wells on location have been put into production. The permitted well bore is producing off of a plunger lift system and with my inspection I did not noticed any annular pressure issues. The production equipment on site consists of the following:

9- 500 barrel production tanks 9- Permitted well bores utilizing a plunger lift system 9- Gas production units equipped with internal heater units and sand traps 1- Low pressure separator 1- Combustor unit 1- Compressor 3- NGL tanks 3- NGL convertor units The site passed preliminary restoration in accordance with ORC 1509.072 A.

0884488499 2014-08-07 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 30 inch diameter casing string. Precision Drilling drilled a 36 inch diameter hole to 100 ft. and set 100 ft. of casing. O-Tex Cementing Service cemented with 310 sacks of class A cement at 15.6 lb. and a yield of 1.20 and had 15 bbl. Of cement returns to surface. Could not enter the GPS readings because the footage of the site has changed. I did contact Columbus to make sure that the permit change had been submitted.
1098436552 2014-08-23 Casing String Witnessed No Violations On the date of inspection I was informed by the company man on site, Russ White, that the Nomac 137 rig had drilled a 17.5 inch diameter hole to a TD of 615 feet. Wyoming Casing Crew then set 612.15 feet of 13.375 inch casing with a weight of 54.5lbs/ft. O-Tex Pumping, LLC pumped 810sks of Premium H cement at 15.6ppg, yield of 1.2 with 2% CaCl, 0.25lbs/sk of Cello-Flake, and 0.4% C-41P. O-Tex pumped 88bbl of fresh water for displacement. 37bbl of cement returned to surface and stayed at surface while I was on site.
1112606381 2014-09-02 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by company man Buzz Glover that the Nomac 137 rig was at a depth of 7982 feet with an 8.25 inch diameter bit. I also witnessed that the French drain underneath where the oil spill was located had been removed and excavated beneath. I was informed that SunPro had taken samples beneath the drain to analyze for Total Petroleum Hydrocarbons (TPH), Oil Range Organics (ORO), and Diesel Range Organics (DRO). Pending the results of the analyses, the trench will remain open with the off site valve closed.
1410292283 2014-09-09 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by company man Buzz Glover that the Nomac 137 rig had drilled an 8.75 inch hole to a total depth of 13,795 feet. 5.5 inch casing weighing 20lbs/ft was set on bottom and the production string was cemented the previous night. At the time of my inspection the Nomac 137 rig was skidding to the next well (4H).
1551738562 2015-05-19 Preliminary Restoration No Violations On the date of inspection I am on site to conduct an inspection for preliminary restoration. The well passes for preliminary restoration per ORC 1509.072(A).
1695473656 2014-08-27 Casing String Witnessed No Violations On the date of inspection I was informed by AEU company man Buzz Glover that the Nomac 137 had drilled a 12.25 inch diameter hole to a total depth of 2,085 feet. 2,024 feet of 9.625 inch casing weighing 36.00lbs/ft (J-55) had been set. O-Tex Pumping, LLC. then pumped 210sk of lead Premium H Cement with a weight of 15.2ppg, a yield of 1.31, 3% gel (Extender), 2% CaCl, and 0.4% C-41P (Defoamer). O-Tex then pumped a tail of 470sk of Premium H Cement with a weight of 15.8ppg, a yield of 1.21, 3% Gypsum, 2% CaCl, 0.2% C16A (Fluid Loss), 0.5% C-45 (Suspension Aide), and 0.4% C-41P (Defoamer). O-Tex then displaced cement with 148.42bbl of fluid. 30bbl of cement returned to surface. When I arrived on site I was notified by the company man that shortly prior to my arrival on site that approximately 25 gallons of motor oil was spilled on the pad outside of containment while servicing a forklift. While I was on site, I witnessed the Nomac workers remove contaminated gravel down to soil and disposed of it in their half round. A small amount migrated into the area of where the french drain was buried. The workers dug down to the PVC pipe and applied organic absorbant and disposed of it in the half round. The company man hired W-Pol Excavating to come out and remove the buried pipe and properly clean the area. W-Pol would be arriving later that night or in the morning to complete the clean up.
2129044382 2016-05-05 Production Wells No Violations I arrived on location, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:
