0438173487 2014-07-22 Drill / Deepen / Reopen No Violations On the date of inspection I spoke with Eclipse Resources Company Man Chad Rice. The Nomac 245 rig is drilling ahead at a depth of 13472 at the time of my inspection. Chad Rice expects to reach Total Depth late today 7/22/14 or early tomorrow 7/23/14. The construction contractor Whipstock is continuing to work along the access road dumping aggregate into the ditch line and installing culverts and sumps.
0720815914 2014-05-19 Casing String Witnessed No Violations On arrival on location Rig crew was running 13 3/8 inch casing to the depth of 389 feet. Halliburton Energy services was prepairing to cement casing to serface. Circulated 9 bbls of cement to surface.
1244471071 2014-07-11 Drill / Deepen / Reopen No Violations On the date of inspection I witnessed that the Nomac 245 was rigging up on the Madzia Unit 10H well. I spoke to the Eclipse Company Man on site for a status update. Nomac 245 plans to re-spud late tonight or early tomorrow morning. The 9.625 inch casing has been set and the rig will begin drilling an 8.75 inch hole for 5.5 inch production casing.

1376406859 2014-03-03 Casing String Witnessed No Violations Butches Rat hole Service drilled a 30 inch diameter hole to 110 ft and set 110 ft of 24 inch diameter casing. Halliburton Cementing Service c/w 290 sks of class A cement 15.6 and 1.22 with good returns. Pick up cement info this day for the job which was set 03/01/2014 when I was out of hours.
1400595879 2014-05-20 Non Well No Violations Eclipse Resources called to inform me that an 18 wheel tractor trailer had overturned leaving their lease road. The truck was hauling drill cuttings from the intermediate section of the well bore. The depth of that sections is from 400 ft to 1500 ft. Inspector Akins was on site and collected photos and tested the cuttings for chlorides. The cuttings were sampled using a quantab titration test strip. It did not register any chlorides. I met with Eclipse Resources Bruce Carpenter and Bill Radford and went over the cleanup details. They are to try and hydrate the cuttings and vacuum them up. The material which can not be vacuumed away will be slowed from release by absorbent booms which are in place.
1400597203 2014-05-20 Non Well No Violations I received a phone call from Josh Angleline with Eclipes Resources that a Waste Management truck hauling drill cutting had overturned at the bottom of the their lease road and the county road. On arrival of location, I found several booms had already been installed in the little stream that travels under the lease road. Bruce Carpenter from the Eclipes environmental department , and Steven Milles with Weavertown environmental group was on site. The drill cuttings were from the intermediate string section, 389 feet to 1560 feet. All fluids from the truck were contained around the truck and no fluids made it to the stream. I took a water sample about 50 feet from the end of the culvert, using Quantab low range and chlorides were not detected. There were no injuries. The truck is being removed and clean up is to begin.
1400615708 2014-05-20 Casing String Witnessed No Violations On arrival of the location I found Aplhia-Hunter rig #9 running 9 5/8 inch casing to the depth of 1547 feet. Halliburton Energy Services will do the cementing. Established circulation, and circulated 25 bbls of cement to the surface. Good returns.

The Nomac 245 rig re-spudded on 7/13/14. The rig is drilling ahead with a 8.75 inch bit at a depth of 5823 feet at the time of my inspection.
1407246458 2014-08-01 Drill / Deepen / Reopen No Violations On the date of inspection I spoke with Eclipse Resources Company Man Ryan Bartholomew. The Nomac 245 was drilling a 8.75 inch hole at a depth of 13,636 feet at the time of my inspection.
1803305514 2016-12-15 Preliminary Restoration No Violations On the day of my inspection I witnessed the lease road in good repair and the gates to access the location were locked. With my arrival on location I witnesses I witnessed all wells on site are now in producing order. The permitted well bore is utilizing natural reservoir pressure for means of production. The production equipment that is on site consists of the following:

2- 500 barrel production tanks 7- Permitted well bores utilizing natural reservoir pressure 7- Gas production units equipped with internal heaters and sand separators 1- Low pressure separator system

The site passed preliminary restoration in accordance with ORC 1509.072 A.
