0993588588 2014-09-17 Drill / Deepen / Reopen No Violations Patterson Drilling Rig # 257 drilled a 12-1/4 inch diameter hole to 6720 ft. and set 6707 ft. of 9-5/8 inch diameter 47 lb casing. Stingray Cementing Service cemented with 869 sacks of lead cement at 12 lb and a yield of 2.18 of class H cement and a tail of 610 sacks of 15.2 lb and a yield of 1.26 of class H cement. Had good cement returns indicated. I was on another job when this job went off. This was a single stage job and they did not run a DV Tool.

1406753013 2014-07-30 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 18-5/8 inch diameter casing string. HAD Drilling Rig # 690 drilled a 24 inch hole to 320 ft. and set 320 ft. of 87.5 lb. casing. The cement head was leaking and had to be torqued in another round or more and then the cement job continued Stingray Cementing Service cemented with 406 sacks of class A cement at 15.6 lb. and a yield of 1.2 Did not get cement returns back to surface. The job will be topped off after the next cement job to cement the # 1 well.

1411471970 2014-09-22 Drill / Deepen / Reopen No Violations Patterson Drilling Rig # 257 is at 14,753 ft and has reached total depth. They will circulate the hole trip out with drill string and begin running 5-1/2 inch diameter casing tonight and cement. I did a check on the containment for the drilling rig and found it did not have any leaks or tears at this time.

1663964773 2016-01-07 Preliminary Restoration No Violations On the day of my inspection, I witnessed the well was equipped with a Cactus Wellhead and a production tree. Production equipment for the well included a sand trap, line heater, vertical separator and a gas compressor. Production equipment that was shared with permitted well 34067212780000 included a unit vapor recovery system, gas combustor, secondary flare stack and 8-400 bbl. tanks that where placed inside proper containment. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1970558002 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains two properly labeled producing plunger lift wellheads. Production equipment includes eight 400 barrel steel tanks in their own polymer lined steel containment, two GPUs, two sand seperators, two vertical seperators, an oil/water knockout, and a combustor .Surface annular pressure is 50 and intermediate annular pressure is 150. Production records are current in RBDMS.
