0438718269 2015-07-15 Production Wells No Violations I received a call from Hal Miller of Gulfport Energy to inform me that a spill and fire had occurred on the Davidson Well Pad and that the fire was out. He advised he found out at 11:55 A.M. Hal called and advised that he would be mobilizing equipment to clean up the spill of less that 20 gallons of condensate and possibly compressor oil. He said that the fire was extinguished by closing off the dump line from the vapor recovery unit feeding the Flare Stack. I informed both Deputy Chief Claus and Region Supervisor Bear while at the Zanesville Region Meeting. I later arrived to find that a Super Sucker vacuum truck was used to pick up 3 inches of dirt and rock that make up the dike surrounding the Flare Stack where the leak had accumulated. The dike is a 15 ft. x 15 ft. area. The limestone and dirt has been vacuumed up and tested with a PID meter to verify the removal of the condensate. When I arrived Hal did the tests to verify the soil was clean and it was later backfilled and re-rocked. The Super Sucker Vacuum truck was to dump the contents into a roll off container which after it was tested would be removed to a landfill.
0827202114 2016-01-07 Preliminary Restoration No Violations On the day of my inspection, I witnessed the well was equipped with a Cactus Wellhead and a production tree. Production equipment for the well included a sand trap, line heater, vertical separator and a gas compressor. Production equipment that was shared with permitted well 34067212790000 included a unit vapor recovery system, gas combustor, secondary flare stack and 8-400 bbl. tanks that where placed inside proper containment. The site passed preliminary restoration in accordance with ORC 1509.072 A.
1147684663 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains two properly labeled producing plunger lift wellheads. Production equipment includes eight 400 barrel steel tanks in their own polymer lined steel containment, two GPUs, two sand seperators, two vertical seperators, an oil/water knockout, and a combustor. Surface annular pressure is 500 and intermediate annular pressure is 1300. Production records are current in RBDMS.
1406562021 2014-07-28 Drill / Deepen / Reopen No Violations HAD Drilling drilled a 26inch diameter hole and set 120 ft of 26 inch diameter casing. It was not cemented because they drilled using a cnncentric drilling method.
1407008641 2014-08-02 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 18-5/8 inch diameter casing string. HAD Drilling Rig # 618 drilled a 24 inch hole to 320 ft. and set 315 ft. of casing. Stingray Cementing Service cemented with 468 sacks of class A cement at 15.6 lb. and a yield of 1. 20 After circulating cement on this well move back to the # 2 well and run in with 1 inch tubing and circulate cement to surface.
1432898490 2015-05-28 Production Wells No Violations At 3:00 PM I received a call that there was a fire on the Davis Pad of Gulfport Energy. I did not have a Davis Well Pad in Harrison Co so I called area Inspectors to advise. I also contacted Hal Miller of Gulfport and he did not know of any fire on any pad of Gulfport but would check to see and get back to me. Brad Burwell of Gulfport Energy called and said that there was a fire on the Davidson Pad and that it was out. There were no injuries and the fire dept. was not contacted. He advised that it was electrical in nature and would get back to me with more detail. Brad did call around 4:15 PM and said that the fire was on a solar panel battery box. The wire to the battery box shorted and the only fire was on the plastic box housing. The fire was not oil or gas related and that a fire extinguisher was used to put out the fire. He also said that repairs had been made and that the wells are back in operation.
1438873449 2015-07-28 Production Wells No Violations I am reviewing the clean up of this site which is now complete from the spill which occurred on July 15 -2015. The spill was from 10 gallons or less of condensate which spilled and also caused a fire on the flare stack. All of the excavated and vacuumed material are on site in a roll off container waiting to be released to the landfill.
1517363065 2014-08-11 Drill / Deepen / Reopen No Violations Patterson Drilling Rig # 257 is moving in today. They are about 50 % on location. Should be 2 to 3 days before drilling begins.
1920945185 2015-04-29 Production Wells No Violations I am checking on this site today to see if there are any signs of chlorides on the pad. I did check two sumps on the pad which had water in them and found them almost below detection levels. The well has all of the equipment in place to produce this well and it appears to be in operation.
