1017770222 2018-03-15 Preliminary Restoration No Violations On the day of my inspection I found pad to contain 5 properly labeled producing wellheads. Wells are equipped with christmas tree style wellheads. Wellhead pressures read: surface 0 psi, intermediate 0 psi, production 373 psi, and tubing 450 psi. Production equipment includes 5 gpuss, 5 sand seperators, 1 heater treater, 1vapor recovery tower, 1 combustor, 2 compressors, and 12 400 barrel steel tanks. Tanks inside metal containment. Pad outslopes are vegetated and stable. Site meets requirements of )RC 1509.072A for preliminary restoration.

Precision Drilling drilled a 30 inch diameter hole to 100 ft and set 100 ft of casing. O-Tex Cementing Service c/w 250 sks of class A cement.

Nomac Drilling Rig # 137 drilled a 14-3/4 inch diameter hole to 2001 ft and set 1995 ft of casing. O-Tex Cementing Service cemented with 290 sacks of class H cement @ 15.6 lb- 1.31yield of lead cement and 940 sacks of 15.8 lb class H cement with a yield of 1.21 of tail cement.
