0064552548 2013-12-30 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling status of Nomac Drilling Rig # 137 and found that they are at 5900 ft making a bit trip. The rig also encountered H2S gas and I advised them to notify each time that they have a reading over 15 PPM - Also advised that the berm along the entrance side of the pad is torn and needs repaired. They are driving on the berm in front of the cat walk and it will need repaired.

0504992656 2018-03-15 Preliminary Restoration No Violations On the day of my inspection I found pad to contain 5 properly labeled producing wellheads. Wells are equipped with christmas tree style wellheads. Wellhead pressures read: surface 0 psi, intermediate 0 psi, production 350 psi, and tubing 200 psi. Production equipment includes 5 gpuss, 5 sand seperators, 1 heater treater, 1vapor recovery tower, 1 combustor, 2 compressors, and 12 400 barrel steel tanks. Tanks inside metal containment. Pad outslopes are vegetated and stable. Site meets requirements of )RC 1509.072A for preliminary restoration.

0903991124 2016-05-05 Production Wells No Violations I arrived on location, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

1283036677 2013-12-19 Drill / Deepen / Reopen No Violations AEP Company Man called me at 5 AM to inform me of the up coming cement job for 1 pm today. I arrived to find out that they had problems with the new air package and are not done drilling. They forgot to call. They called at 3 pm and will be done running casing and ready to cement at 8 pm. I arrive at 8 pm to find that they could not get casing in the well . They are circulating every joint to bottom. I waited several hours to see if casing would go and it did not. I was out of hours at that time so I contacted Inspector Grimm to have him cover the job which now has turned into Friday morning.
1386271818 2013-12-05 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 24 inch diameter casing string. Express Drilling drilled a 30 inch diameter hole to 100 ft and set 100 ft of casing. O-Tex Cementing c/w 250 sks.
1389368272 2014-01-06 Drill / Deepen / Reopen No Violations I am on site today to check on the plug back status of the strat section of the well. Nomac Drilling Rig # 137 drilled a 8.75inch diameter hole to 8260 ft and then set a cement plug from 8260 to 6645 ft. O-Tex Cementing Service c/w 640 sks of class H cement @ 17.5 lb and a yield of .94
1389701316 2014-01-13 Drill / Deepen / Reopen No Violations I am on site today to pick up cement information. I found that Nomac Drilling Rig # 137 has set and cemented 5-1/2 inch diameter production casing. They will begin moving the rig to the 3-H well and should start drilling Tuesday.
