1223158497 2016-01-11 Preliminary Restoration No Violations On the day of my inspection, I witnessed the well was equipped with a production tree. Production equipment for the well included a sand trap, a gas production unit and a line heater. Production equipment that was shared with permitted wells 34067212280000, 34067212290000, 34067212300000, 34067212320000, 34067212330000, 34067214310000, and 34067214320000 included a vapor recovery system, 5 gas compressors, a gas combustor and 12-400 bbl. tanks that where placed inside proper containment. The site passed preliminary restoration in accordance with ORC 1509.072 A.
1428065068 2015-01-18 Casing String Witnessed No Violations I was on location to witness the cementing of the 13.375 surface casing. Nomac Rig #73 spudded the surface hole on 1/18/15, drilled a 17.5 hole to a depth of 531, and Wyoming Casing ran 13.375 surface casing to a depth of 521. O-Tex rigged up, tested the lines, and pumped 30 barrels of gel and 10 barrels of water ahead of the slurry. They then mixed and pumped 105 barrels of cement slurry, dropped the plug, and displaced the plug to the collar with 73 barrels of water. The plug landed at 250psi and the pressure was increased to 800psi and held for 5 minutes. The pressure was released, and the floats held with 0.5 barrels flowing back to the truck. 28 barrels of cement circulated to surface and weighed 15.2lbs/gal on the pressurized scales. 1449655997 2015-01-22 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #73. They are currently tripping in the hole with a new 8.75 air hammer. The previous bit was ruined when a booster went down and a rig hand failed to notify the driller. Their current depth is 6,406 and they will drill to the mud-up-point of 7,266, load the hole, and switch to the 8.5 bit to drill the curve and lateral. All drilling equipment is on containment and the location is in good condition. No violations.
1567094438 2016-02-18 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the wells are in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 Low pressure separator tower/Vapor recovery system unit. 5 compressors

There was approximately 2 to 3 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.
