0128021497 2014-12-17 Casing String Witnessed No Violations On the day of my inspection I found that Nomac Rig 72 had installed 13 3/8 casing in a 17 inch wellbore, to the depth of 562 feet. Schlumberger will cement casing to surface. Established circulation and circulated 37 bbls of good cement returns to surface.
0180633051 2014-09-10 Casing String Witnessed No Violations This well was spud on 9/9/2014. Universal cemented 120 of 20 94# casing with 400 sacks of class A cement. 20 bbls of good cement was returned to surface. See the casing ticket for the details.
2076341830 2016-02-17 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 4 Producing wells 4 Sand separators

2 Horizontal heater-treaters. 3 Compressors 2 NGL tanks There was approximately 4 to 5 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.
