0085941695 2013-12-19 Drill / Deepen / Reopen No Violations I am on site today to witness the running of the 30 inch diameter casing string. It will not be cemented in place. HAD Drilling set 197 ft of 30 inch diameter casing inside a 30 hole.
0195931128 2013-12-18 Drill / Deepen / Reopen No Violations HAD Drilling Rig # 618 is drilling the 30inch diameter conductor hole and will set 197 ft of 30 inch conductor. It will not be witnessed because it is not cemented in place.
0806122515 2015-05-19 Preliminary Restoration No Violations On the date of inspection I am on site to conduct an inspection for preliminary restoration. The well passes for preliminary restoration per ORC 1509.072(A).
0963436952 2016-09-07 Production Wells No Violations On the day of my inspection I witnessed the lease road gate was locked and the lease road in adequate condition. The permitted well is producing off of natural reservoir pressure. RBDMS records indicate that the well is in producing order. The production tubing is being monitored using a digital Psi gauge placed at the top of the well head. At the time of my inspection the production tubing gauge read approximately 1357 psi. Fluids and gases could be heard transferring from the wellhead to the flow line. Indicating the well is producing. I was unable to capture the pressure readings of the surface casing and intermediate casing due to the fact it was not clear what position the valves were in. The gases coming from the tanks were being flared off.

Production equipment on the well pad consisted of the following. 4- 400 barrel production tanks 1- Condensate compressor station. 4- Gas Production Units with built in line heaters and sand separators 1- Flare stack. 4- Producing wells utilizing natural reservoir pressure.

1029222231 2014-01-21 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 18-5/8 inch diameter casing string. H & P Drilling Rig # 287 drilled a 22 inch diameter hole to 513 ft and set 500 ft of casing. Halliburton Cementing Service c/w270 sks @15.8 lb yield of 2.31 had 20 bbls of good cement returns.

1513015560 2017-12-11 Production Wells No Violations On the day of my inspection I found this pad containing 4 producing wellheads. Wells heads are on a separate pad from production equipment. Production equipment includes four 400 barrel production tanks, four GPUs,one compressor, and flare. Well is showing 50 psi on the surface casing and 0 psi on intermediate casing. Valves are open and production records are current.
