The well remains shut down while the other 3 wells on the pad are in operation. The clean up of the defoamer spill continues and is around 95% complete. Rick Trippel is working on details of the clean up with Mark Scrobacz. Mark is to send the results for the last soil analyses.
1264303890 2014-08-20 Production Wells No Violations On the date of my inspection I accompanied ODNR Inspector Mark Moreland to meet with Hess personnel regarding the lightning strike that occurred at one of their production tanks. We also met with Hess and Envirotrac personnel regarding the spill clean up efforts. Ron Trivisonno and Rick Trippel with ODNR were also on site to take over the handling of the spill clean up. Refer to Mark Morelands report for further details.

1373376650 2013-07-08 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 18-5/8 inch diameter casing string. H&P Drilling Rig # 284 is running casing but is having trouble getting casing to bottom. I remained on site for as long as they were making progress. It appears that they will not be able to run casing to total depth.
1373485162 2013-07-10 Casing String Witnessed No Violations I am on site today training with Mark Moreland to witness the running and cementing of the 18.625 inch diameter surface casing. H&P Drilling Rig # 284 drilled a 22 inch diameter hole to 410 feet and set 385 feet of casing. Halliburton cemented with 220 sacks of Class A cement @ 15.6lb and 1.18 yield. During our cement job a thunder storm with high winds and rain shut down the operation until it passed. The storm lasted for more than 2 hours.
1373939885 2013-07-15 Casing String Witnessed No Violations I was contacted early this morning to inform that casing would be run and cemented at around noon today. I arrived on site to witness the running and the cementing of the 13-3/8 inch casing string. H &P Drilling Rig # 284 drilled a 17-1/2 inch hole to 1930 ft. I arrived with Doug Bennett to witness the cementing operation. During the running of the casing a hydraulic line broke and sprayed a floor hand. The operation was shut down for several hours until repairs could be made. We left at that time to return later.
1376926404 2013-08-19 Drill / Deepen / Reopen No Violations I arrived today for a site inspection and found H &P Drilling # 284 drilling at 4800 ft on the vertical section of the well. I also checked on the cement info and rig schedule.
1390326593 2014-01-21 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling status of H & P Drilling Rig # 284 and found that they are at 18,345 ft and are just reaching total depth. They will make several wiper runs back to bottom and then trip out to run 5-1/ inch casing in the 8.5 inch diameter hole. Should be ready to cement casing Friday or Sat.
1408138042 2014-08-15 Production Wells No Violations On the date of inspection I was on site to witness Hess contain their spill and cover the contaminated area in case of rain. The Hess company representative on site was Renee Cowell, EHS & SR Supervisor. The contractor on site was Envirotrac with representatives Dennis Furmanek and Diane Webb. Envirotrac and Hess attempted to use an old liner they had stored to cover the contaminated area. The liner was found to be in poor condition so a new one will be delivered the following morning. Envirotrac performed field screenings of soils in the ditch area with a PID and readings ranging from 0.0ppm to 6390ppm. They were unsuccessful in attempts to procure a geoprobe to delineate the contamination range as well as an excavator to dig a sump in the ditchline to prevent down gradient contamination. They plan to have both pieces of equipment on site as well as utilities marked first thing in the morning. Envirotrac and Hess did not show up on site until 14:00.
1408201719 2014-08-16 Production Wells No Violations On the date of inspection I witnessed Hess and Envirotrac establishing containment of the spill on site and the delineation of the extent of contamination. Hess representative on site was Brian Lockwood. Envirotrac Project Specialist on site is Dennis Furmanek. At the time of my inspection I witnessed that a heavy duty liner had been placed across most of the known contaminated area including between heated separators. The up gradient culvert and the down gradient culvert have been sealed with silicone and plywood to prevent water from leaving site in the event of a storm. A pit approximately 3 feet by 3 feet was excavated within the drainage ditch in front of where the spill took place. At the time of my inspection Envirotrac was in the process of delineating the contaminated area using a geoprobe. The geoprobe contractor is Geo Environmental Drilling Company from Pittsburgh, PA.
1408396652 2014-08-18 Production Wells No Violations On the date and time of my inspection there had been no significant changes to the site of the spill since my report on date 8-16-14. There were sections of the liner over the contaminated area that had been cut and glued back together in areas where test borings were performed on 8-16-14.
1593644138 2013-07-15 Casing String Witnessed No Violations I am on site tonight to witness the running and the cementing of the 13-3/8inch diameter casing string. H & P Drilling Rig # 284 drilled a 17-1/2 inch diameter hole to 1930 ft. and tried to set 1925 ft of casing but could not get it to bottom. Pull out last joint of casing and set 1880 ft. Halliburton C/W 830 sks of class A cement @ 15.6
1620961742 2014-08-14 Production Wells No Violations I received a call from Hess Ohio Development Rene Cowell that a spill of Defoaming agent had leaked at the production facility. The Defoaming agent is very hazardous and requires a Haz Mat crew to respond. The crew responding to the spill is Weavertown and they are to mobilize and be on site today around noon. Myself and Inspector Brandon Trigg went to the site and met with the Weavertown Crew along with Rene Cowell and Brian Lockwood both from Hess Ohio. They reported that a spill of 20 to 150 gallons could have occurred. The spill went undetected because the line carrying the Defoaming agent was below ground level and out of site. It could have leaked a day or more before being detected. When Inspector Trigg and I arrived we found that the area in front of the line heaters contained most of the spill in the dirt under and next to the line heaters and in the road ditch in front of the Defoamer tanks. We witnessed Weavertown remove soil under the Defoamer tanks once they had been removed and in the road ditch. Booms and absorbent pads were placed in the ditch and in front of the line heaters to absorb some of the free defoamer while still liquid. After a very short time, digging, on site was stopped once it became clear that it was too close to high pressure lines and electrical lines. More info is to be gathered and a plan on how to continue developed. Samples are to be collected and shared with ODNR by Weavertown.
1630758246 2014-08-14 Production Wells No Violations Arrive on site today to meet with Weavertown Consulting Group to see when and how they are going to contain and remediate the spill site. Hess Ohio Development has had a defoamer leak out of containment and into a road ditch in front of the production pad. Rene Cowell reported a spill of the Hughes Defoamer at around 150 gallons or 3.6 barrels of DF03043U defoamer. They are to review the spill and submit a plan for remediation to ODNR as soon as possible. They are to collect samples to determine the extent of the area contaminated and share the results with ODNR.
1906631879 2017-08-31 Preliminary Restoration No Violations On the day of my inspection I found the Cadiz B pad seperated between a production equipment pad and a wellhead pad. Five properly labeled chritsmas tree style welheads are on location. Production equipment includes three 400 barrel steel condensate tanks, one 400 barrel brine tank, five line heaters, one seperator, flare, and vapor recovery system. Production equipment is fenced and locked and access the wellheads is also restricted by fence. Production records are current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A.
