0000632009 2015-01-06 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by Chesapeake consultant, Rodney Bean, that the 5.5 inch Production Casing was cemented the previous night. This was the last well scheduled to be drilled on the Wright pad. The Patterson 261 rig was in the process of rigging down and will be moving out of Ohio.
0314100058 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 4 compressors 2 NGL tanks The site passed preliminary restoration in accordance with ORC 1509.072 A.

0827559450 2015-05-28 Preliminary Restoration No Violations On this date I am on site to conduct an inspection on the well site for Preliminary Restoration. The well passes for Preliminary Restoration per ORC 1509.072(A).
1419541974 2014-12-23 Casing String Witnessed No Violations On the date of inspection I am on site to witness the surface casing cemented to surface. I was informed by Chesapeake consultant, Gary McKee, that the Patterson 261 rig drilled a 17.5 inch diameter hole to a total depth of 590 feet. Wyoming Casing Service set 13.375 inch casing weighing 54.50lbs/ft to 580 feet. O-Tex Pumping, LLC pumped 540sk of Class H cement, 15.6 pg, 1.2 yield, 2% CaCl2, 0.4% C-41P, and 0.25lbs/sk. O-Tex displaced cement with 82bbl of fresh water, bumped plug, floats held, 24bbl of cement returned to surface and stayed at surface while I was on location.

The rig was at a depth of 10,256 feet. Gary McKee informed me that the rig would most likely reach Total Depth later in the week and would be rigging down to move out of Ohio.
