0382959525 2014-11-17 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by Chesapeake consultant Rodney Bean that the Patterson 261 rig was currently under maintenance and would be drilling a 12.25 inch diameter hole through the shoe after repairs were made. The 13.375 inch surface casing was set the previous afternoon.
0717420312 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 4 compressors 2 NGL tanks The site passed preliminary restoration in accordance with ORC 1509.072 A.

0768187041 2014-11-24 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by Chesapeake consultant, Joe Hebert, that the Patterson 261 rig was currently drilling an 8.5 inch diameter hole 60 degrees into the curve at a depth of 8, 219 feet.
1416099996 2014-11-15 Casing String Witnessed No Violations On the day of my inspection I found Patterson Rig 261 running 13 3/8 inch casing to the depth of 606 feet. O-Tex will be doing the cementing. The pump truck broke down and they had to call in a different pump truck and a different cement truck. All trucks arrived on location at 7:00 am on the 16th. The second pump truck has broke down. Leaving location.
1416501577 2014-11-19 Casing String Witnessed No Violations At the time of my inspection I found that Patterson Drilling rig 261 had drilled a 12.25 hole to a depth of 2116. They were running new 9.625 casing.
1642374086 2015-05-28 Preliminary Restoration No Violations On this date I am on site to conduct an inspection on the well site for Preliminary Restoration. The well passes for Preliminary Restoration per ORC 1509.072(A).
1733429206 2014-11-19 Casing String Witnessed No Violations At the time of my inspection I found that Patterson Drilling rig 261 had new 9.625 casing ran to a depth of 2106. O-Tex Pumping broke circulation and cemented the casing to surface with 770 sacks of class H cement. Good circulation was maintained throughout the job, with no significant increases in pressure. There were approximately 33 barrels of cement circulated to surface. O-Tex landed the plug at 1100 psi and bumped it up to 1609 psi. They held the pressure for 5 minutes. The pressure had fallen to 1594 psi. when they released it.
