0406641770 2014-12-08 Drill / Deepen / Reopen No Violations On the date of inspection I was on site to speak with the Chesapeake consultant about the complaint that had been issued from an adjacent landowner. Patterson 261 is the rig on site. The landowner, Amy McAfee, had stated that she was able to detect odors of windex and rotten eggs at her home and that there was white dust accumulating on her lawn furniture over the past few months. I spoke with Chesapeake consultant, Rodney Bean. He stated that the white dust could have been from the lime that is used to mix with drill cuttings in order to solidify them and haul them away. It is likely that the wind had carried over small amounts of lime to the adjacent landowners property. Mixing lime with drill cuttings is unavoidable in order to haul drill cuttings away. When I have been on site over the past few months for inspections, I had not noticed any odors outside of the normal operations. Rodney Bean informed me that they had not had any H2S alarms go off over the course of drilling. I called the phone number provided by Amy McAfee on two occasions and left messages explaining what I had found and that she was free to call me back to discuss. The Patterson 261 rig was currently drilling an 8.5 inch diameter hole at a depth of 2448 feet. There is one well left to be drilled after the current well.
0869576143 2015-05-28 Preliminary Restoration No Violations On this date I am on site to conduct an inspection on the well site for Preliminary Restoration. The well passes for Preliminary Restoration per ORC 1509.072(A).
1404318595 2014-07-02 Casing String Witnessed No Violations Express Drilling drilled a 26 inch diameter hole to 120 ft and set 120 ft of 20 inch diameter casing. Express Cementing Service cemented with 230 sacks of class A cement at 15.6 lbs and a yield of 1.18. Circulation was established and they had 10 bbls of cement returns.
1407426351 2014-12-04 Casing String Witnessed No Violations On the date of inspection I was informed by Chesapeake consultant, Rodney Bean, that the Patterson 261 rig had drilled a 17.5 inch diameter hole to a depth of 608 feet. Wyoming Casing Services set 599 feet of 13.375 inch diameter, 54.50lbs/ft casing in the hole. O-Tex Pumping, LLC pumped 540sk of Class H cement, 15.6ppg, 1.2 yield, 2% CaCl2, 0.4% C-41P, and 0.25lbs/sk of Cello Flake. O-Tex displaced cement with 85bbl of fresh water. 17bbl of cement returned to surface and stayed at surface while I was on site.
1537628859 2014-07-02 Casing String Witnessed No Violations On the date of inspection, I accompanied ODNR Inspector Mark Moreland to witness the running and cementing of the 20 conductor casing. Express Drilling drilled a 26 hole to a depth of 120 and set 120 of 94# casing. Express Cementing Service cemented with 230sks Class A at 15.6# and yield of 1.18. Company man present on site is Brett Farnsworth. Express Energy Services established circulation in hole before cementing and received 10bbl of cement back to surface. Cement stayed to surface while I was on site.
1675772115 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 4 compressors 2 NGL tanks The site passed preliminary restoration in accordance with ORC 1509.072 A.
