0178456643 2016-03-08 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells on site are producing off of a plunger lift sytem. Permitted well 34067211840000 has not been drilled. RBDMS indicated that the wells on site are in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 3 Producing wells 3 Sand separators

3 Line heaters 1 Vapor recovery tower 1 combustor The site passed preliminary restoration in accordance with ORC 1509.072 A.

0274084459 2013-12-12 Production Wells No Violations I received a call from Chris Anderson of HAL Associates about a spill which occurred at this pad. The low pressure valve on the dump side froze and broke. It allowed two gallons of condensate to spill onto the rocks in front of the left side of the production tanks dike. The rock has been excavated and contained in a roll off container. The well is back in operation.
0952149345 2013-10-09 Preliminary Restoration No Violations On the day of my inspection the well passed preliminary restoration according to 1509.072A. Well was flowed back for 48 hours last week, tank battery construction should be completed by tomorrow.
1283420912 2014-01-13 Drill / Deepen / Reopen No Violations While I was driving past the pad I witnessed a dozer and a track hoe rebuilding the slope on the right side of the pad toward the entrance. I found out that the slope had a slip and is being rocked and graded. There was no other problems on the pad.
1370261019 2013-06-03 Casing String Witnessed No Violations I am on site today to witness the running and cementing of the 26 inch diameter casing string. Redstone Drilling drilled a 36 inch diameter hole to 135 ft and set 135 ft of casing. Stingray Cementing Service c/w 610 sks.
1375983307 2013-08-08 Drill / Deepen / Reopen No Violations I am on site today to check on Sidewinder Drilling Rig # 60 and found that they are moving to the Inhurst Pad in Belmont Co. They ran 17,965 ft of 5-1/2 inch diameter casing. See cement ticket for more information. Company man on site was Scott Treadway.
1385563885 2013-11-27 Production Wells No Violations I am on site today because of a spill reported to me by Chris Anderson. He advised that between 20 to 40 gallons of condensate had been released onto the pad near the production tanks. The release occurred when a 2inch 1000 lb check valve failed. The release was condensate which covered an area of 30ft x 40 ft long. Clean up was started minutes after the well was shut in. A water truck was on site at the time of the release and clean up started at that time. During my inspection today a vacuum truck along with a pressure washer is on site and the area is being flushed and vacuumed. There are absorbent pads down which are collecting any condensate which may float after power washing. Last the contaminated rock and soil is to be contained in a roll off container and removed to a land fill.
1979191591 2013-12-02 Production Wells No Violations I am on site today to check on the minor spill of a quart of condensate which was spilled after the 40 gallon spill. The spill has been cleaned up and the rock and contaminated soil remains on site in a roll off container.
