0078242964 2017-09-06 Preliminary Restoration No Violations On the day of my inspection I found the Darla wellpad to contain four properly labeled texas style wellheads. 100 psi on surface casing and no pressure for intermediate casing. Production equipment includes two horizontal seperators, four gas production units, three 400 barrel steel condensate tanks, three 400 barrel steel brine, a 50 barrel steel brine tank, and a combustor. Tanks are in their own steel containment. Production records current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A.
0369827812 2014-03-31 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that clean-up of oil based mud spill is completed and approved. Nomac Drilling Rig #310 is rigging down moving to the Brothers Pad.
0473019532 2014-03-30 Drill / Deepen / Reopen Violations Noted On the day of my inspection I was notified by Kris Andersen of ALL Consulting that there was a spill of oil based mud while rigging down was taking place. One plug was inadvertently left off on the discharge line of two frac tanks on the evening of 3/29/30 with ice/solids in line from contents of tanks. Today at noon solids control hand Bob Baldwin was backing in a vacuum truck when the ice/solids plug broke free spilling approximately 12 total barrels of oil based mud. 10 barrels inside containment and due to a tear in the liner underneath the drill mats 2 barrels left containment. Upon my arrival I witnessed that drill mats were being lifted up and power washed off and cedar chips were being applied outside containment to soak any residual oil based mud. Owner/Operator shall pull up remainder drill mats isolate and vacuum inside containment and then roll back liner and clean up any pools of contamination. Once pools are cleaned Owner/Operator shall dig down with backhoe and remove contaminated stone and notify inspector so area can be re-inspected prior to adding clean fill.
0585017639 2015-11-10 Production Wells No Violations We are on site to conduct a visual inspection of the production pad because of prior spills that had occurred. Inspector Moreland and I found that the pad is maintaining lush vegetation and shows no signs of spills leaks or contamination. There were no signs of off site impact in regards to prior spills.
0687135898 2013-09-12 Drill / Deepen / Reopen No Violations Met Deputy Chief Bob Worstall on site with Ross Kirtley to review improvements made to the liner and the containment system in the cellar. The wells which are not being drilled are closed off and the well being drilled is contained from the Tin Horn to the containment liner. Bob and I also reviewed the spill which occurred 09/03/2013. The containment system has been improved to catch more of the chlorides. During our inspection we found that the fluid running over to be captured has almost stopped. The containment system to capture the chlorides is to remain in place until ODNR determines it is safe to remove it. Deputy Chief Worstall advised Ross Kirtley that he would pass along the findings of our inspection to Chief Simmers and he would make the determination if drilling could continue.
0713885669 2013-09-04 Drill / Deepen / Reopen Violations Noted I am on site today to see what has been done to remediate the spill which occurred 09/03/2013 I found that no equipment has been on site. The only thing done has been to dig a small shovel wide ditch and use a vacuum truck to pull up the fluid it generates. Next I collected two low range chloride quantab samples. Both of the readings peaked the low test strips. Hal Miller tested the same fluid and found that the range exceeded the 6000 PPM range. I contacted Deputy Chief Worstall and advised of my findings. He advised me to call Kyle Baldwin and Rick Triple and see if they could lend some assistance. I remained on site to meeting with the above people and fill out spill reports and download photos for Columbus. After meeting with Blake, Rick, and Kyle we all agreed more information and results from the lab would be needed to decide on remediation. Blake and Rick Triple are to develop a short term plan and go over that with Gulfport and Nomac 310.
0733813640 2013-08-26 Drill / Deepen / Reopen No Violations Nomac Drilling Rig # 310 is at 2149 ft and just starting to dust the 12-1/4 inch diameter hole.
1028865461 2013-08-08 Drill / Deepen / Reopen No Violations I was advised that Gulfport is to begin moving Nomac Drilling Rig # 310 onto this pad later today. The pad has conductor set and the containment is set up for the rig to arrive.

1378138604 2013-08-29 Drill / Deepen / Reopen No Violations Nomac Drilling Rig # 310 is at 1275 ft and should be ready to set casing Friday night.
1378305284 2013-09-04 Drill / Deepen / Reopen Violations Noted I am on site today to review remediation measures for the spill which occurred 9/03/2013. I found that a vacuum truck has been vacuuming fluid since the spill occurred around noon on Tuesday. The fluid which is coming from under the liner and off the rock pad continues off the site 200 ft then down over the hill. The vacuum truck has been on site and has pulled up almost 100 bbls of fluid at this time. I contacted Deputy Chief Worstall to update on my findings. He said that I should request assistance from ODNR Engineers and have Kyle Baldwin collect samples for the lab. I collected quantab samples which show chlorides over 6000 PPM. Next I collected photos and will merge them with a spill report for Columbus. The Company Man on site when I arrived was Randal Stogner and was leaving for his days off at 7:00 am. There is no cement information on site and I requested a copy from the arriving Company Man Matt Knight.
1394288806 2014-03-08 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied I witnessed that Nomac Drilling Rig #310 was drilling an 8.5 inch diameter lateral hole at depth of 10436 feet with an estimated td of 16057 feet and were tripping out of the hole due to tool failure. Completed walk around with company man Matthew Knight discussed continuing testing of water in secondary containment.
1426035595 2013-08-19 Drill / Deepen / Reopen No Violations I was contacted by Nomac Drilling 310 to advise me that they would be cementing surface casing on the 17th. They will be setting surface later today but did not call. Met with Anthony Miller on site and advised that they need to contact me when they are going to cement. They need to call when they the job has been cancelled as well.
1433848511 2013-08-22 Drill / Deepen / Reopen No Violations Nomac Drilling Rig # 310 is drilling a 17.5 inch diameter hole and they are at 1017 ft and will drill to 2170 ft to T.D this section of the well. Should be Friday or Sat morning.
1655763780 2016-01-15 Production Wells No Violations I am on location today after receiving a phone call from Brad Burwell on 1/14/2016 who informed me that a spill had occurred on site. Brad stated the spill had occurred from a glycol injector malfunction and that approximately 15 gallons of glycol was spilt on site. He also stated that the spill was cleaned up promptly after it had occurred and that it was contained, without ever leaching off the well pad. The affected area from the spill measured approximately 5 feet 3 feet. Glycol is used to start the fire for the gas combustor. The glycol injector did not shut down due to mechanical failure, causing the injector to overheat and melt. On the day of my inspection I witnessed that the spill was properly accessed. Visually there were no evident indications that a spill had occurred and no indications of pollution or contamination on site. After concluding my inspection, I then called the Ohio Oil and Gas Emergency Response Line to inform them of what I just had learned.
1962174081 2014-03-03 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR inspector Randy Akins, I witnessed that Nomac Drilling Rig #310 has completed their skid and have completed BOP testing. Nomac Drilling Rig #310 will trip in the hole and drill out the 9.625 inch float shoe @ 7416 feet later tonight.
2127107279 2013-10-22 Drill / Deepen / Reopen No Violations I am on site today to check on the clean up status of the spill which occurred 09/04/2013 - I found that Nomac Drilling Rig # 310 has moved to the next well but is no drilling at this time. The spill has been monitored since the spill date and the chlorides are now down to a normal range. The containment system is still in place and there are no further violations at this time. I did notice during my inspection that the berm was damaged on the containment and was advised it is to be repaired within the hour. Contacted Doug Bennett and advised of my findings
