0333229163 2013-05-31 Drill / Deepen / Reopen No Violations I received a call from Anthony Miller of Gulfport Energy to advise me that there may a problem with this drilling site. He called to advise of a possible contamination problem occurring on this drilling pad. He was not sure of the source of the contamination or how big of a problem it was. I was later contacted by Deputy Chief Bob Worstall asking me to conduct an inspection of this site. I am working with Kyle Baldwin to collect fluid samples and photographs to send to our lab . Kyle collected water samples at 40.19040 -081.18607 and at 40.19025 -081.8118626 . A quantab titration test for chlorides was conducted and the field results indicated that the chloride range was in excess of 28,000 PPM. Kyle collected all of the samples and then transported them to our lab in Cambridge for further testing. There has been no corrective action taken at this time until results from the Cambridge lab are complete.

Production equipment included a sand trap, a line heater, a separator, a compressor unit, a vapor recovery system, 9-400bbl tanks and 1-500bbl tank all within containment and a gas combustor. I found that the liner has two places where it has collapsed. There are two water trucks on site and I advised them to pump off the fluid. I advised Nick Berelsman to repair the collapsed sections. Where the liner collapsed it also will need to be patched together. I Remained on site until the fluid was pumped off and the liner repaired.
1211570958 2016-12-02 Production Wells No Violations I am on site today to perform a routine status check of the well bore and the well location. On the day of my inspection I witnessed the lease load was in good repair and the gates for the location were locked. After inspecting the well bore I discovered no high annular pressures and that the well was in producing order. With the inspection of the location I discovered that Gulfport is utilizing the site to hold cinders for their lease roads. Also with the inspection I did not discover any indications of pollution or contamination.

1370347259 2013-06-03 Drill / Deepen / Reopen Violations Noted Met with Deputy Chief Bob Worstall and Supervisor Tom Hill on site. The inspection was for Bob to see the amount of contamination and try to understand how the contamination occurred. The contaminated soil is to be removed and replaced with clean fill . The containment plastic is to be removed and the area below excavated to a point where there is no contaminated soil. The rock can be stored and put back in place after the contamination has been removed. A sump area with a liner shall be installed below the rock area to catch any remaining contamination which may run off of the rocks. The fluids contained in the sump will be removed to an approved U.I.C Injection site until contamination is below 250 P.P.M. Vegetation shall be established in all of the effected area outside of the rocked pad.
1371061861 2013-06-06 Drill / Deepen / Reopen No Violations My inspection today is to see how the containment system functioned in collecting runoff from the contaminated drilling pad. I found that the containment is in place and still has some fluid running under the liner on the containment. I advised Nick Berelsman that fluid was under the liner and he is to have them vacuum it out and set the liner back in place. Next I drew up a grid system on the pad to try and determine the amount of contamination. I had the backhoe dig down through the rock pad and below the fabric liner. When I was deep enough to see fluid I collected samples and ran a low range quantab titration test on the samples. I was able to determine that a portion as large as 75 % of the pad had chloride contamination above the low end limits which is above 636 P.P.M.

1470514256 2013-06-26 Drill / Deepen / Reopen No Violations I am inspecting the site today because of the big storm which rocked the area dropping 4 inches of rain. I arrived to find that the sump area of the control dike has collapsed. The entire dike is full of fluid and there are two places that need repairs. I contacted Chad Shaffner and advised. I returned later in the day and found that the fabric in the sump has been repaired and that the fluid has been pumped off. The containment system is back in operation.
