0072139370 2014-05-21 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Dylan Pendleton, I witnessed that Nomac Drilling Rig #43 drilled a 17.5 inch diameter hole to the depth of 570 feet. Wyoming Casing Crew ran and set 555 feet of 13.375 inch 54.5#/ft J-55 casing. Schlumberger was on site and gained circulation with 25bbls of gel followed by 10bbls of fresh water and cemented casing with 472 sacks of Class A 15.6ppg, 1.22cuft/sack cement. Had good returns of 20bbls, plug was landed at 221psi and pressured to 839psi for 5 minutes and the floats held.
0509134986 2017-08-15 Preliminary Restoration No Violations On the day of my inspection I found the Ericson pad to contain two properly identified wellheads, two 500 barrel steel brine tanks with identification contained in a metal dike, no oil tanks, and two sand seperators. There was 100 psi annualar pressure on the 9 5/8 inch diameter casing and none on the 13 3/8 inch diameter casing. Pressure on well was approximately 650 psi. Well is equipped with a christmas tree style wellhead. Site passes preliminary restoration according to ORC 1509.072A.

1401200415 2014-05-27 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #43 drilled an 8.5 inch diameter lateral hole at a depth of 10081 feet with a planned td of 16107 feet. Production casing was being offloaded, Intermediate casing job went well and site inspection revealed no violations.
1622950518 2014-04-27 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 20 inch diameter casing string. Enviro Drilling drilled a 30 inch diameter hole to 115 ft and set 115 ft of casing. O-Tex Cementing Service cemented with 418 sack of class A cement @ 15.6 lb and a yield of 1.18 O-Tex was 3 hours late to the job then still had to wait on a water truck.
1695748175 2014-06-09 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Brandon Trigg, I witnessed that Nomac 43 had completed their kick plug (KOP) from 7780 feet to 7205 feet on Friday and are currently sidetracking the KOP at 7573 feet with plans of landing their curve on Wednesday. Site inspection revealed no violations.
