0075234875 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains four properly labeled producing wellheads. Production equipment includes twelve 400 barrel steel tanks in their own polymer lined steel containment, one 50 barrel steel tank, four GPUs, four sand seperators, and a combustor. Surface annular pressure is 50 and intermediate annular pressure is 0. Production records are current in RBDMS.
0089788491 2013-10-07 Production Wells No Violations I am checking on this site today to see what type of activity is going on. They had been fracing on site the last few days but all of the equipment in gone.
0225300683 2017-08-22 Production Wells No Violations On the day of my inspection I found this pad to have four producing wells, twelve 400 barrel steel tanks, four gas production units and four sand seperators. Surface and intermediate annulas has no pressure.

0309901991 2013-06-25 Drill / Deepen / Reopen No Violations I am returning later today to check the containment system which was to be completed last night. I found that it needs to be expanded because it is not wide enough to catch the entire contents. I contacted Chad Shaffner and advised. I then met with Triple A services and went over the revised containment with Brad Allar. We marked off the area to be included and reviewed how to tie the system together. I remained on site while modifications were made and advised that they could begin drilling now that the containment is in place.
0316045938 2013-04-12 Drill / Deepen / Reopen No Violations I am on site today to witness the running and the cementing of the 26 inch casing string. Redstone Drilling Service drilled a 32 inch diameter hole to 116 ft and installed 116 ft of casing. Stingray Cementing Service c/w 290 sks. The job was running behind and I was not able to witness the cementing of the casing. I also had a problem with RBDMS and could not open the file to complete my report.
0800205211 2013-08-22 Production Wells No Violations I am on site to check on the containment system which is being installed around the entire well pad. It does not appear that any more work has been completed on the containment. There is a backhoe on site which is digging up the area where the secondary containment was set up to contain fluid leaking out from under the containment while the well was drilled.
1173141438 2013-04-24 Casing String Witnessed No Violations I am on site tonight to witness the running and cementing of the 16 inch diameter casing. Yost Drilling # 3 has drilled a 20 inch diameter hole to 430 ft and set 405 ft of casing. Schlumberger Cementing Service will cement to surface with 483 sks of class A cement. During the installation of the casing the power tongs broke down and another one will takes several hours to arrive.
1322188697 2013-04-25 Casing String Witnessed No Violations I am on site again to witness the running and cementing of the 16 inch diameter casing string. During the installation of the casing string early last night the power tongs broke down and I left at 2:00 am. The casing was installed to 405 ft in a 430 ft hole and cemented by Schlumberger Cementing Service with 483 sks of Class A cement. Had 25 bbls of good cement returns.
1343467810 2013-12-10 Preliminary Restoration No Violations On the day of my inspection I witnessed that the well had been completed and was in production. I found no identification at the wellhead and flow back sand was still on site in half rounds which were covered and in containment. Sand has been tested and is in the process to be removed from location. Made call to Adam Ferett Gulfport Production Superintendent about identification and he is getting an updated ID list and will address the issue.
1372165142 2013-06-24 Drill / Deepen / Reopen Violations Noted I am responding to a spill which was reported by Gulfport Energy. I was advised that after the rig was moved to the next well the rig was washed down and that the rig crew saw fluid running under the containment. I arrived and found that fluid which had chlorides in the range of 2660 P.P.M. was running from under the pad containment. It is running along the entire length of the pad where the flare stack and half round tanks are located. I collected samples and transported them to our lab in Cambridge.
1372166982 2013-06-25 Drill / Deepen / Reopen No Violations I arrived today to find that the secondary containment which was to be established below the drilling pad toward the 1/2 round tanks and flare stack, is not in place. The containment which is set up at this point will not keep more than 1/2 of the pad from leaking off site. I met with two company men from Gulfport and went over what would be needed to ensure containment . Triple A is to return to complete the remaining containment controls.
1372290539 2013-06-26 Drill / Deepen / Reopen No Violations I am on site today to check on the containment system which was installed to catch chloride contamination. I found that the containment system had fluid running under it and that the liner had a hole. Advised Chad Shaffner of the liner leak and need to pump off fluid from under the liner. I also was informed that 10 to 15 gallons of Aqua Foam drilling soap had been spilled on the rock pad. There is a vacuum truck on site and it is vacuuming the soap up during my inspection. Before I left the site I made sure the soap was cleaned up and the liner repaired.
1645885201 2013-08-26 Production Wells No Violations I am on site today to check on the containment system installed on this pad. I found that they placed a limestone cap stone down on 80% of the drilling pad and that the secondary containment system I installed after a brine spill has been removed and the area backfilled and graded.
1781481024 2016-04-21 Production Wells No Violations This well has corrected all of the violations and is now back in operation. The pollution and contamination has been removed
