1062338149 2016-02-19 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 6 Producing wells 6 Sand separators

2 Horizontal heater-treater that was disconnected 1 combustor 1 low pressure separator/ Vapor recovery system 1 compressor 2 NGL tanks that were disconnected at the time of my inspection There was approximately 2 to 3 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1394204080 2014-03-06 Casing String Witnessed No Violations On the day of my inspection I found that Nomac Drilling rig 70 ran 553 of new 13.325 casing. O-Tex Pumping cemented the casing to surface with 520 sacks of class H cement. Good circulation was maintained throughout the job, with approximately 25 barrels of cement returns.
1394461210 2014-03-10 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Nomac Drilling rig 70 was making up the down hole motor and switching to fluid in preparation to start the curve. They were at a depth of 7682. The permit was posted in the company mans trailer and I found no leaks or spills around the drill site.
1916019203 2017-09-22 Preliminary Restoration No Violations On the day of my inspection I found the NM Harrison pad to contain six properly labeled wellheads. Wells are equipped with chritmas tree wellheads. Gauges show no surface casing pressure and 400 psi on intermediate. Production equipment on the pad includes six vertical seperators with gas production units, one heater treater, five 400 barrel steel condensate tanks, three 400 barrel steel produced water tanks, vapor recovery system and a combustor. Tanks are in their own steel containment. Production records current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A.
