0132758265 2016-02-26 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 2 Producing wells 2 Sand separators

2 Horizontal heater-treaters 1 Low pressure separator/ Vapor recovery system 1 Combustor The site passed preliminary restoration in accordance with ORC 1509.072 A.

0611839762 2015-11-05 Production Wells No Violations Myself and Inspector Moreland are on site to visually inspect the left side of the down hill slope of the production pad that has been reconstructed. The slope was part of a slip that occurred after the well was in production. Today we found that the slope was properly reconstructed and established good vegetation cover.

1367835190 2013-05-06 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 9-5/8 inch diameter casing string. Nomac Drilling Rig # 70 drilled a 12-1/4 inch diameter hole to 1840 ft and set 1826 ft of casing. Schlumberger Cementing Service c/w 690 sks of class A @ 15.6 per lb and 1.2 yield.
1452268545 2018-08-13 Production Wells No Violations On the day of my inspection I witnessed this pad to contain two properly labeled producing wells. Surface and interediate annualr pressures are 0. Production records are current in RBDMS. Production equipment includes three 400 barrel steel produced water tanks, five 400 barrel steel condensate tanks, two sand separators, two GPUs, a compressor, and a combustor. Tanks are inside their own steel containment.
