0131582478 2013-09-30 Preliminary Restoration No Violations On the day of my inspection the well was in production and the site passed preliminary restoration according to 1509.072A.
0360689250 2013-04-01 Drill / Deepen / Reopen No Violations Met with Deputy Chief Bob Worstall and Supervisor Rocky King on site. Review drilling and producing on the pad. Next review the 50 bbl drip tank set up to see if the spacing needs to be changed. It appears to be far enough away from the compressors on this location not to be a problem.
0845222540 2013-03-12 Drill / Deepen / Reopen No Violations Patterson rig # 480 ran 1695 of 13-3/8 54.5# R-3 casing. Stingray Cementing Services cemented the string from top to bottom with 1364 sx of Class A cement. There were good cement returns to surface. See the casing ticket for details.
0907782395 2016-03-07 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells on site are producing off of natural reservoir pressure. Permitted wells 34067211040000 and 34067211050000 have not been drilled. RBDMS indicated that the wells on site are in producing order.
The production equipment on the pad consists of the following:

14 400 bbl. Production tanks 4 Producing wells 4 Sand separators

4 Line heaters 1 compressor 1 Vapor recovery tower 1 combustor 1 30bbl. drip tank The site passed preliminary restoration in accordance with ORC 1509.072 A.

1362798367 2013-03-08 Drill / Deepen / Reopen No Violations I am on site today to witness the running and the cementing of the 18-5/8 inch diameter casing string. Patterson Drilling Rig # 480 drilled a 24 inch diameter hole to 440 ft and then set 427 ft of casing. Patterson Drilling could not get casing to bottom and it became clear I could not witness the casing cement job and stay within the time constraint.
1379512208 2013-09-18 Production Wells No Violations I am on site today to check the well pad and outside for low vegetation. I found most of the pad and just off the pad has good vegetaion. There is a large pile of soil with pit liner mixed in it on site.
1387027270 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains four properly labeled producing wellheads. Production equipment includes five 400 barrel steel tanks in their own polymer lined steel containment, four GPUs, four sand seperators, four vertical seperators, and a vapor recovery unit. Surface annular pressure is 0 and intermediate annular pressure is 0. Production records are current in RBDMS.
