The production equipment on the pad consists of the following:

1343923817 2012-07-31 Drill / Deepen / Reopen No Violations I arrived on site to check on the drilling status of Patterson Drilling Rig # 259. I found that the rig was at 0 and would begin drilling within the hour or around 11 AM. The conductor was already drilled and cemented by a Top Hole rig. Patterson will be drilling a 17.5 hole to set 13 3/8 casing. I passed out my ODNR cards and advised the Company man to contact me at the same time as he contacts his cement crew. He advised that it would be about 8 hours prior to cementing.
1344254263 2012-08-05 Drill / Deepen / Reopen No Violations DNR Supervisor Gene Chini was on site 08/03/2012 to witness a Cement Bond Log on the 13 3/8 inch casing. Chief Simmers had ordered a Cement Bond Log of the 13 3/8 inch casing because the Operator had failed to notify ODNR before that casing was cemented. Supervisor Chini witnessed and approved the Cement Bond Log. Chesapeake was contacted and the Compliance Notice was resolved at that time. Patterson Drilling Rig # 259 will be setting the 9 5/8 inch casing string today. I am on site to witness the running and cementing of the casing.
1399953014 2012-08-01 Drill / Deepen / Reopen No Violations I arrived to check on Patterson Drilling Rig # 259 and found that the drill string had been tripped out of the hole. A logging truck was on site to log the 17.5 inch hole. I found out that there had been a mistake on drilling depth and the hole had not been drilled deep enough. Patterson is to go back in and drill to 610 and no deeper than 640 ft. I again passed out more ODNR cards to the Company man (Josh) on site. I also passed out a 3 page sheet with all of the Inspectors, Supervisors, Central Office staff phone contact information. I again spoke with the Company Man on site and advised that I be contacted along with the cement crew. I also advised that I had been on Nomac Rig # 39 and that they had not contacted me. I was advised that that would not occur this time.

2019199297 2012-08-02 Drill / Deepen / Reopen Violations Noted I am inspecting this site today because I felt that this job should have happened last night. When I arrived I found that the 13 3/8 casing had already been installed and cemented. Next, I spoke to the Company Man on site and he informed me that I had been notified 48 hours ago. I replied that I had been on site 3 times and during each time I passed out my ODNR card and advised that I be contacted at the same time as the cementers. Each time I was advised that I would be contacted and not to worry. This is the second time this week that a rig drilling for Chesapeake has not called me before installing casing and cementing. I contacted Chief Simmers and he advised me that the Patterson # 259 be shut down and that they be required to submit a copy of the cement bond log to Tom Tomastik before drilling continues.
