Previous inspection report reveals that well was plugged and buried, verified by Inspector Joe Zollars in Novemeber 2016. No plugging report exists in RBDMS. Final restoration cannot be completed until plugging report is retrieved.
0759125365 2016-05-02 Plug / Plug Back No Violations I called Mark Clark after not receiving the plugging records that I had requested. He stated that he and Robert Griffin were in the process of gathering all the correct paper work that I had requested. I stated that I would like that information sent to my ODNR email address, also in the conversation I asked Mr. Clark if this was the only well Robert Griffin had plugged without proper ODNR standards, he stated to the best of his knowledge, yes it was. To further conduct my investigation I decided to call Robert L Griffin directly. Mr. Griffin stated that the well was plugged when he hired a driller to come in and drill out the hole after the open-hole section of the well bore had collapsed. Once the driller had completed the drill out, the driller noticed that the hole had watered out and he decided then to grout the well to surface. I stated to Mr. Griffin that I am requesting the paper work on how the well was plugged including cement tickets, service contactors, any and all documentation related to plugging of the Crothers 03-23 well. Mr. Griffin stated that he would send me the paper work to my ODNR email address.

1253076139 2016-04-18 Plug / Plug Back No Violations Called Mark Clark a plugging contractor used by Coal Gas Transportation LLC to inquire if Robert Griffin planned on plugging the Crothers 03-23 well since a 2nd plugging permit had been issued on 12/07/2015 and the well was not plugged according to ODNR records. Mark Clark stated that to the best of his knowledge the well was plugged in the summer of 2015. Mr. Clark thought maybe Robert Griffin was confused and did not fill the paperwork out correctly.
1356258414 2016-04-18 Plug / Plug Back No Violations After my phone conversion with Mark Clark in regards to the Crothers 03-23 well condition. I did a well site inspection and I discovered that the well bore was not visible and a pile of dirt was in place on top of where the surveyed coordinates indicated the well should be, it is evident that the well is no longer existing. After my inspection, I then called Mr. Clark back, Mr. Clark then stated that Robert Griffin was confused on the paperwork requirements and accidently reapplied for a plugging permit after the well was plugged. I informed Mark Clark to have Robert Griffin send me the paper work pertaining to the plugging of the Crothers 03-23 well, including all third party contractors and cement tickets showing his plugging procedure. Mark Clark said he would forward the information on to Robert Griffin.

1467729456 2016-06-10 Administrative Inspection No Violations I received a e-mail in response to Inspector Zollars call to Mr. Bob Griffin of Coal Gas Transmission asking for a copy of the records of the cement ticket, name of the company which cemented the well and the wireline company. Bob stated in the e-mail that I was on site 10/2012 when the well was plugged and that he had given me all of the cement tickets and wireline plug information. I was able to complete a records check and verify that I had not plugged this well on any date.

1474309742 2016-09-27 Plug / Plug Back Violations Noted Upon completion of reviewing all documentation submitted pertaining to the plugging and abandoning of the Roger & Goldie Crothers #03-23 well (Permit No 34-067-2-0978) I have discovered the following violations; 1.) There was no valid plugging permit issued during the reported plugging of this well in 2010, 2.) The Owner/Operator failed to properly notify the Division, 3.) The plugging report did not reflect what method of plugging was used to plug the well, 4.) The plugging material used was not approved by the Chief of the Division, 5.) Failure to submit plugging report within 30 days after plugging the well bore, 6.) General Health and Safety, R.C. 1509.23 as it relates to the plugging of wells for protection of public health or safety to prevent damage to natural resources. The Owner/Operator shall submit a written drill out and plugging schedule to Joe Zollars at 2207 Reiser Avenue S.E., New Philadelphia Ohio, 44663 within 15 days, drill out and plug the well from bottom of the 7 inch diameter casing to surface within 30 days. The Owner/ Operator will be notified if the schedule is approved or denied.

On the day of my inspection I discovered the well was cut three feet below ground level and had a tin plate with the well number placed on-top of it. After removing the plate, I found what appeared to be good cement at the top of the casing. After inspecting the cement I then placed water inside the casing on top of the cement. The water did not bubble, indicating no signs of gas travel from the old production reservoir to the surface. After completing my inspection, I witness Mark Clark placing a new steel plate (with the wells permit number) over top of the well and filling it in. I then called Mike Moreland and informed him of my inspection.

After inspecting the well bore I placed a call to Mark Clark who informed me that he plans to dig up the well bore on November 28th, 2016. I will be on site to witness the digging up of the well bore.

Previous inspection reports indicate well was plugged and cement was verified inside of casing. Site is vegetated with no signs of well. Site meets final restoration per ORC 1509.072B.
