1458564111 2016-03-10 Plug / Plug Back No Violations Appalachian was running in to check depth and run a bond log. I accompanied Joe Zellers on day of inspection. Company man was Mark Clark.
1460375749 2016-04-08 Plug / Plug Back No Violations Ran 4-1/2 casing to approximately 265 and Cemenco Services filled the well to the surface with cement. Good returns to the surface. Pulled the casing and topped off with cement back to the surface. Rigged down and moved the rig off. Left cement at the top of the hole.
1724775015 2016-03-10 Plug / Plug Back No Violations Appalachian Well Service is on site to run a CCL log inside the 4.5 inch diameter casing to indicate if there is a packer set on the backside of the casing, in order to discover what will need to be done to plug this well bore. After Appalachian ran the CCL log there was no indication that a packer was set on the 4 inch diameter casing. After the log I placed a call to Mark Moreland who informed me that a service rig will have to pull the casing in order to plug the well or a bon-log will need to be run to indicate if there is cement on the backside of the casing. I was accompanied by inspector Pat Shreve on the day of my inspection.
1827353745 2016-04-07 Plug / Plug Back No Violations I am on site where I witnessed Davis Well Service used a service rig to unset the packer and pull out 615 feet of 4.5 inch diameter surface casing. Appalachian Well Service is on site to set a wire line bridge plug inside the 7 inch diameter surface casing in preparation for plugging this well. Appalachian hit an obstruction in the hole at 315 feet. The last collar for the 7 inch diameter pipe was found at 300 feet. The bridge plug was then set at 285 feet. The well will be filled with cement on a later date.
1863635813 2016-04-19 Plug / Plug Back No Violations On the day of my inspection I witnessed the well location has been restored, but vegetation has yet to be established. I will return at a later date to indicate if the site can pass final restoration.
2101445252 2015-03-09 Production Wells No Violations I am on site accompanied by inspector Mark Moreland to meet with Mark Clark to review the plans on what will need to be done to plug the well bore. After approximately an hour of waiting we realized Mark was not going to show up. We then conducted a site overview and Mark informed of what shall be done to plug the well bore.
