0040311735 2018-07-09 Production Wells No Violations On the day of my inspection I found this well equipped with a walking beam pumping unit that it not connected to the rods in the well. Purpose of this inspection is to follow up on complaint number 5419 about lease road erosion. Pumping unit has a natural gas engine. Valve on tubing is closed. Casing valve open to domestic gas line. Production equipment includes 2 poly brine tanks, a 210 barrel steel tank, and horizontal seperator. One tank is cracked from a tree falling onto it and could potentially over flow with a heavy rain. The tanks are ~150 west of the seperator. All equipment within containment. Well and production equipment is poorly labeled. No erosion noted on lease road. Production records are current.
0442872385 2015-10-09 Production Wells Violations Noted I had a call from the landowner of the well Mr. Bill Corcoran of 84555 Mcgill Rd Deersville, Ohio - phone # 330-388-7791 to advise me that there was an H2S leak at his home and at the well head near his home. He said that it was leaking and he was concerned about other homeowners safety. He did know the name of the well and I contacted the Operator Bob Didi of D&D Energy Service to advise I had received a call that his well was leaking gas and H2S . I contacted Inspector Akins and together we went to the well to investigate. Upon arriving at the well it was discovered there was no easy way to approach the well because of the steep terrain and thick underbrush. Our safety meters went on alarm for methane and LEL but did not register any H2S. During our inspection we found that there were several gas leaks on fittings and lines that need repaired. I advised Bob Didi about the leaks and the need to repair them along with the culvert which has signs of being washed out and bypassed by stream running through it. Bob is to send his pumper to correct the leaks on his next scheduled round Saturday. The culvert will need equipment and most likely a dump truck with topsoil. Bob is to work on that this next week and advise me when it has been repaired.
0450606902 2016-09-01 Production Wells No Violations On the day of my inspection following up on complaint number 5419 where a landowner is concerned about the condition of the lease road and culvert pipe on lease road, I found the lease road and culvert pipe were in adequate condition with the exception of on section where a 20 by 10 foot area of erosion was discovered. I witnessed the well was set up to be utilizing a pump-jack for means of production. The well consists of 4.5 inch diameter production casing, 2.375 inch diameter production tubing and rods, polish rod and stuffing box. At the time of my inspection the pump-jack was removed from the well and the production valves were placed in the open position. Production equipment for the well bore consisted of a horizontal separator and a 2 inch diameter meter run. A domestic gas hook-up is plumed into the separator. RBDMS records indicate that the well is in producing order. After my inspection I called the landowner and well owner; they came to an agreement that the well owner Bob Diddi would fix the erosion on the lease road.
1064895420 2015-10-13 Production Wells No Violations I returned today to check on the repair status of the gas leaks at the well head on several fittings and lines. I found that the leaks are fixed and my gas detector did not go on alarm today. Repairs to the culvert are to begin after the operator is able to find and mobilize equipment. Bob Didi, the owner, said he will need a dozer, backhoe and a dump truck.
