0104914689 2016-06-03 Production Wells No Violations Tim McConaughey called to advise me that the 1 1/2 inch diameter tubing was stuck and that it would need shot off. He wanted to ask how high they could shoot it. He called back to ask if they shot the tubing off could they shoot the perfs thru the tubing further up the hole. I advised that unless it was a last ditch effort they could not shoot thru the tubing. Tim called again to ask since they did get the tubing shot off could they pull the tubing out and shot the 4 1/2 inch diameter casing while the shooters were still on location. I ask if they planned not to set the bottom plug and just pull up the tubing to 4150 ft. and drop out the bottom to shoot. Tim said he planned to not set the bottom plug today and yes pull out the tubing to 4150 ft. so they could shoot, then run back to bottom and be ready to plug Monday at 9:00 or after.
0237150778 2016-05-20 Production Wells No Violations I arrived on site today to find that this well has been stripped down and ready to plug. Tim McConaughey said that they planned to move Chipco Well Service on soon to plug.
0587065042 2016-06-07 Plug / Plug Back No Violations Chipco Well Service ran in the hole to tag the top of plug # 2 and found the top of the cement at 3726 ft. They ran back in the hole with the ripping tool to rip the 4 1/2 inch diameter casing and could not get the tool below 1200 ft. After several attempts Tim McConaughey called Appalachian Well to shoot off the 4 1/2 inch casing. They shot the casing off at 1527 ft. but when they tried to pick it up it parted. Advised run back inside the 4 1/2 inch casing with 1 1/2 inch tubing and set plug # 3 Cemented with 120 sacks of class A cement at 15.6 lbs. and a yield of 1.18 Then pull out the tubing and pulled out the parted 4 1/2 inch casing. Pulled out 300 ft. of casing and went back in with 1 1/2 tubing to set plug from 200 ft. to 0 ft.
0943131729 2016-07-06 Final Restoration No Violations I am checking this well after it has been plugged to see if the restoration is complete. The site has been graded and vegetation has been established on most all of the wellsite. The restoration will not be approved today because there are riser pipes and valves remaining on site. There is a set of valves that could be for gas transmission but the riser and valves connected to it will need removed.

1057364915 2015-07-27 Production Wells Violations Noted I arrived on site to find a rabbit well with a horizontal separator and a 210 bbl production tank. The well has no pressure on it and all of the valves are open. The chart on the meter box has not been changed in years and the area is overrun with vegetation. I talked with Troy Wells area Production Supervisor and he confirmed that the well has not been in operation since 2012. The well shall be plugged within 180 days.
1461768055 2016-04-27 Production Wells No Violations There has been no work done to get this well ready to plug. It remains idle with all of the valves closed . Contact Tim McConaughey at 330-903-4782 and Tim advised me that the well is scheduled to be plugged in the next 15 to 30 days, weather permitting.
1465301760 2016-06-06 Plug / Plug Back No Violations Chipco Well Service is plugging this well today. They have 1 1/2 inch diameter tubing in the well at a depth of 5718 ft. Chipco set the bottom plug at 5718 to 5318 ft. cemented with 60 sacks of class A cement at 15.6 lbs. and a yield of 1.18 Prior to setting plug # 2 they pumped 65 bbl. Of freshwater to try and establish circulation and could not. I advised that if the plug was set without circulation plugging could not continue until the next day when the top of the plug could be tagged. Set the plug and Chipco pulled out all of the 1 1/2 inch tubing. After all of the tubing was out they started a hot tap on the 8 5/8 inch diameter casing. There were not valves on the surface casing and they did not want to try and unscrew the plug if there was pressure on the annular space. They decided to hot tap the surface casing and put valves on. After the valve was opened it was determined that there was no pressure on the annulus.
