0865499582 2014-01-13 Production Wells No Violations I am on site today to check on the status of the well. The well was to have been plugged in Nov. of 2013- Due to problems with H2S plugging Nicks Well Plugging could not get to it before the weather turned bad. I contacted Nicks Well and he may be able to plug in this area some time in Feb. and this well is on the list to be plugged then. The tank battery and the well is still hooked up and has not been stripped down at this time.
1195617225 2016-01-15 Final Restoration No Violations I am on site today accompanied by the land owner Robert Puttman. Robert called me a day prior to the inspection in concerns of a live gas line that was left on his property after the well was plugged. We then arranged to meet at the area of concern the next day. On the day of my inspection I found that the gas line is a sales line that ties into multiple wells and then goes into a compressor station. Robert stated that he would be logging his property in two months and the gas line in placed in the area where he wants to stack the logs to be hauled out. The line is currently buried approximately 1 foot underground, however when the logging operation comes in they will need level the ground where the line is at. Where the line is placed now, the leveling out of the area cannot be completed till the line is moved. In accordance the 1501:9-10-05 (A) The rule states that the line must be buried at least twenty four inches below the ground surface; unless a statement is filed with the Chief of the Division following on of the exceptions. I will further investigate by calling S&S Energy to see if an exception was filed to the chief, if not I shall ask them to bury the pipeline deeper, so Mr. Puttman can safely complete logging operations .
1366724813 2013-04-23 Production Wells Violations Noted I had a call from Mr. Putnam and he advised me that the well had been idle for two years and that he wants it plugged. I conducted a well inspection and found that it is idle. All of the equipment is in place to produce the well. I contacted Jason Scott and he told me he plans to plug the well and will apply for a plug permit this week. The time line for plugging the well is the next two to three months with weather as a factor. The well is off the road and across a low bottom so it will need to be some what dry to get equipment in and out. I will contact Mr. Putnam and advise that the well is to be plugged. 330-854-3124
1383922868 2014-03-19 Plug / Plug Back No Violations This well has been plugged to surface. I am on site today to check on the well and casing status. I found that the pits are pumped off and closed. The casing has been cut off and I did not find any leaks. The area still needs graded and seeded along with the removal of the production tanks. Final restoration can not be passed at this date.
1394730946 2014-03-13 Plug / Plug Back No Violations Nicks Well Plugging is on site today to begin plugging this well to surface. OBriens Safety Service is also on site because the well has H2S gas. Readings of 113 PPM were recorded when the well was opened to begin plugging. The well has 1-1/2 inch diameter tubing in the well and capacity of the tubing was pumped to make sure it was clear of the rabbit. Then Appalachian was called to shot off the tubing seat and shoot at the Newburg and the Lime. Then tubing was ran back to 5690 ft. Time ran out to set plugs today so it will begin Friday Morning at 7:30 am
1394819460 2014-03-14 Plug / Plug Back No Violations I am on site today to plug the well back to surface with Nicks Well Plugging Service. Thursday we tried to set the bottom plug thru tubing and found that the rabbit was in the tubing seat. Called Appalachian Wwll Service to shoot off the tubing and then pull up and shoot the casing at the Newburg and Big Lime. Today Nicks has tubing at 5690 ft and set the bottom plug thru 2-3/8 inch diamter tubing. Pull up and set the Newberg plug and the Big Lime thru tubing. We found out that the 4.5 inch casing has a hole in it at 1100 ft and we may not be albe to get back in to set the Surface casing plug. I advised that the casing be shot again at 1600 ft and then pull out all of the tubing to finish the top plug. Plugging will continue on Monday.
1395064124 2014-03-17 Plug / Plug Back No Violations I am on site today to witness Appalachian Well Service shoot the 4-1/2 inch diameter casing so that the plug can be set below the surface casing. Nicks Well Plugging has 1-1/2 inch tubing at 1600 ft and the casing was shot at 1628 to 1618 ft. Set # 4 plug from 1628 to 1428 ft and pull all of the tubing out. Rip 4-1/2 inch diameter casing at 1000 ft and pull all of it out and run back in hole with 1-1/2 inch tubing to 200ft and set # 5 plug from 200 to 0 ft and circulate cement to surface.
