0688705643 2018-02-01 Production Wells No Violations On the day of my inspection I found an earthen containment with a 210 barrel steel tank, horizontal seperator, and concrete brine vault. The production equipment has been sold to the landowner but the concrete brine vault must be cleaned and removed prior to doing a final restoration. Tank is identified. Compliance notices 1479233221 and 1479910123 previously issued can be passed as well has been plugged.
1161001891 2016-12-21 Production Wells No Violations I am on site today accompanied by Inspector Randy Akins to perform a follow up inspection on a compliance issue. On the day of my inspection I discovered the well is still shut in and H2S readings of around 118 parts per million were collected on the 4.5 inch diameter well head.
1479233221 2016-11-15 Production Wells Violations Noted I am on site today to conduct a pre exempt transfer inspection. With the conduction of my inspection I discovered the permitted well bore is utilizing a plunger lift system for means of production. The casing program for the well consists of 8.625 inch diameter surface casing, 4.5 inch diameter production casing and 1.5 inch diameter production tubing. The plunger is set up to be operated by a motor valve or with manual valves. Upon my arrival at the well head an odor of H2S could be smelled and gas could be heard coming from the well bore. After inspecting the well head I discovered severe corrosion on the valves that controlled the well and also severe corrosion on the production tubing and production casing above their well heads. I then gathered gas readings with my Industrial Scientific Tango TX1 hydrogen sulfide meter and received H2S readings at 375 ppm around the well head. Production equipment for the well consists of a 140 barrel production tank and a horizontal separator. The production equipment was placed in an earthen dike that measured approximately 40*40 feet with an average height of 2 foot around the perimeter of the dike. Also placed in the earthen dike was the production equipment for permitted well number 34-067-2-0356 (Sayre #1) which consisted of a 210 barrel production tank and a horizontal separator. Over the duration of my inspection of the well head and the production equipment I did not witness any signs or indications that a domestic hook up was present or that any domestic gas was being used. Proper identification was placed on the production tank and at the well head. The well owner/operator shall submit a written plugging schedule to Joe Zollars at 2207 Reiser Ave. S.E. New Philadelphia, Ohio 44663 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
