0025323757 | 2018-10-02 | Plug / Plug Back | No Violations | On the day of my inspection I witnessed a rig onsite set up over the well. No rig hands onsite. Pat Shrieve had called and said that the rig hands walked off location and was unclear as of why. Well is supposed to be plugged but plugging has not started. Tubing is open to atmosphere. Earthen pit has been excavated and lined. |
0671230742 | 2018-10-03 | Plug / Plug Back | No Violations | On location with Inspector Pat Shrieve to witness the bottom plug. A 400 foot plug was pumped by Formation Cementing and the rig crew was unable to trip tubing because of rig issues and cemented it in place. See Pats report for further information. |

-foot cement plug at 8450-8050 feet. They are running regular class A cement at 1.18 yield and 15.6 ppg. They are displacing with 32.6 barrels of 10 ppg of salt water. The rig started pulling 2 3/8-inch tubing and blew an air hose. They shut down for 45 minutes and started pulling about 4 joints at 32 feet per joint. The rig had blown air hoses again and this was over 1 hour. The cement started setting and hung the tubing in the well. | |
1242632747 | 2018-10-10 | Plug / Plug Back | No Violations | On the day of my inspection I witnessed Formation Cementing pump a 400 foot plug using 215 sacks of class A cement. Weight of 14.8 and yield of 1.36. 4.5 inch diamter casing pulled up to 1780 feet. Cement displaced with 21 barrels of water. 100 feet of plug is in 11 inch diameter open hole and 300 foot of plug is in 11.75 inch diameter casing. |
1254565034 | 2018-10-09 | Plug / Plug Back | No Violations | The inspection is conducted under plugging operations. Dylan Pendleton had assisted me on the plug job. The Oriskany / Big lime plug was set from 4860-4560 feet. Formation Cementing services had set plug with 145 sacks of regular class A cement at 2 % gel. The displacement was 72 barrels of 10 lb./gallon of salt water. There was good circulation but the well kicked off and used well control methods to successfully place the plug. |
1326633870 | 2018-10-09 | Plug / Plug Back | No Violations | On the day of my inspection I was accompanied by Inspector Pat Shreve. Formation cementing pumped a plug and rig pulled 4.5 inch diameter casing. See inspector Shreves report for further information. |

1446448985 | 2018-06-18 | Production Wells | No Violations | The inspection is conducted under hot and dry conditions. The temperature is 90 degrees Fahrenheit. I assisted Dylan Pendleton and found that the well is shut in and idle. The well has not produced and has no production lines and production equipment on location. The well is equipped with a large flange 10,000 psi casing head and a 5000-psi valve that is closed. The flanged tubing hanger is closed with 5000 psi valves. The landowner Nellie Schaney and family confronted Dylan and I while we were on location. They wanted to know who gave the authority for us to be on location. I stated that under 1509.03 (C) we have the authority to enter lands that are public or private. I stated I will give her contact information to company. 6-19-18 I gave Nellie Schaney phone number to Larry Murrey. |
1499340834 | 2017-07-06 | Production Wells | No Violations | On the day of my inspection I found this well equipped with 11.75 inch diameter casing, 8.625 inch diameter casing, and 4.5 inch diameter casing. Well has no identification. On 7/5, call was made to Brian Rushe (724-720-2590) at Exco who said that it is on their plugging list. RBDMS shows no production records. |
1529333880 | 2018-06-18 | Production Wells | No Violations | On the day of my inspection I was accompanied by Inspector Pat Shrieve. Purpose of this inspection was for Pat to view the well and try and see if there was any pressure on it. Tubing valve could not be opened and we were unable check pressure. Operator is planning on plugging this well next month. Well has a large wellhead. Casing and tubing valves are closed. No production equipment onsite. Landowner Mrs. Schaney may not want well plugged and was not amicable. |
1572401865 | 2018-10-10 | Plug / Plug Back | No Violations | The inspection is conducted under plugging operations. Unlimited service rig had casing pulled up to 1780 feet. Formation Cementing set bottom of casing plug from 1780---0 with 70 sacks of regular class A cement at 2 % gel. After pulling all the 4 1/2 inch casing out the well was topped off from 150-0. Cement did not drop back. |
