0140342296 2019-05-23 Preliminary Restoration No Violations The purpose of my inspection was a status check on preliminary a restoration. The pad was constructed with disturbed areas restored in the year 2014. In the year 2019 two additional wells were added to the pad. The cuttings from the two cellars and two conductor boreholes were stockpiled on the South side of the pad with vegetation established. The pad slopes appeared to be stable. The well was being prepared for flowback and the wellhead was being assembled. The production equipment was being hauled on to location and arranged on pad. Identification signs and drilling permits were posted at entry. The location meets the requirements for preliminary restoration as per the Ohio Revised Code 1509.072 (A).

-inch diameter intermediate borehole. Ascent personnel conducted a safety meeting and then conducted a walk around tour of the rig and pad. 9.625-inch diameter intermediate casing was evaluated on the pipe rack and two more trucks with loads of 9.625-inch casing were parked on the lease road.

0321004823 2019-04-29 Completion Testing No Violations The purpose of my inspection was a status check on completion operations. During my inspection a workover rig was rigging up on the 5H well to drill out frac plugs and to install production tubing. Sound walls were being removed from location.
0740698923 2019-03-18 Drill / Deepen / Reopen No Violations The purpose of my inspection was a status check on drilling operations. During my inspection Patterson 584 was drilling at 11,338-feet within the lateral section of the production borehole. The kick off point before the curved section was at 6864-feet and the landing point after the curved section was at 8185-feet. On 3/15/19 the 40 pounds per foot 9.625-inch diameter intermediate casing was run and cemented at 6750-feet in two stages to surface. The post cement job was reviewed with the Ascent company man, Michael Burton and 40 barrels of cement returns were established. The 5.5-inch diameter production casing was being hauled onto location and placed on the pipe racks.
0824619014 2019-02-15 Casing String Witnessed No Violations Precision Rathole drilled a 36-inch diameter borehole to 100 feet in depth and installed a 30-inch diameter conductor casing with welded joints. Water was encountered during the drilling process. The conductor weighed 118.65 pounds per foot. During my inspection a pre-job safety meeting was held with all personnel on location and Petroset cementing rigged up on location and pumped 315 sacks of class A cement with 2 percent calcium chloride and displaced with 70.5 barrels of fresh water. Cement returns were established and were used to cement the cellar floor to create a liquid tight cellar.
1370625373 2019-03-10 Casing String Witnessed No Violations The purpose of my inspection was to witness the cementing of the 13.375-inch diameter surface casing within the 17.5-inch diameter borehole. The casing was set at 1356 feet from the rig floor and 3.8 feet of stick up above the rig floor. During my inspection a safety meeting was held with all personnel on location. Following the safety meeting Schlumberger pumped 25 barrels of gel, 10 barrels of fresh water, 1372 sacks of class A cement, and 200 barrels for displacement. The cement was weighed in pressurized mud scales confirming the weight of 15.6 pounds per gallon. Estimated 65 barrels of cement returns established and weighed at 15.2 pounds per gallon
