0255070224 2019-05-01 Completion Testing No Violations The purpose of my inspection was a status check on completion operations. During my inspection Halliburton was on location completing hydraulic stimulation operations on the 6H well. The 8H well was equipped with a frac stack with a flow cross, wing valves, and a goat head. 68 stages have been completed.
0722341056 2018-10-11 Drill / Deepen / Reopen No Violations During my inspection Pioneer 77 was waiting on cement to cure. The 9.625-inch diameter intermediate casing had been cemented into place with 268 barrels of cement at 6450 feet in depth. 44 barrels of cement returned to surface.
0769764572 2019-01-17 Completion Testing No Violations The inspection revealed that the frac- head is on the well and are under toe prep operations. The well is being monitored by a wireless method and fiberoptic ran on the 5 1/2 inch casing. The wells have heat being blown back in the cellars, the base of the well head is covered up. The frac is scheduled for 2-26-19.

During my inspection the well was equipped with a work-over gate valve. The 13.375-inch diameter surface casing had 200 pounds per square inch, the 9.625-inch diameter intermediate casing had 0 pounds per square inch, and the 5.5-inch diameter production casing had 0 pounds per square inch. The well was awaiting hydraulic stimulation operations. The location meets preliminary restoration requirements written within the Ohio Revised Code 1509.072 (A).
1715954701 2019-03-04 Completion Testing No Violations The purpose of my inspection was a status check. During my inspection the location was being prepared for hydraulic stimulation operations. The well was equipped with a frac tree. An aboveground water storage tank had been assembled on the Northeast corner of the pad.
2044785028 2018-08-08 Casing String Witnessed No Violations The purpose of my inspection was to witness the cementing of the conductor. The conductor was 30-inches in diameter and weighed 118.65 pound per foot. Petroset was on location with 1 bulk cement truck and one pump truck. The borehole was 36 inches in diameter and was drilled to 100 feet in depth. Water was encountered during the drilling process at 70 feet. The 30-inch diameter conductor was set on bottom of the borehole at 100 feet. Petroset plumbed up to a 2-inch diameter threaded nipple that was welded on the top plate of the conductor and pumped 315 sacks of class A cement weighing 15.6 pounds per gallon and 75 barrels of fresh water for displacement. 1 foot of cement returns were used to seal the cellar floor. The cellar was 10 feet in diameter and 8 feet deep.
