0355349966 2019-01-14 Completion Testing No Violations On the day of my inspection I talked to company man Chuck Davies on the progress of the work. The well has had frac plugs drilled out and is waiting on placing in production.
0522478711 2018-12-28 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. There were a total of 7 wells on pad, three remain temporarily abandoned, and four had just completed hydraulic stimulation operations and are awaiting drill-out and flow back operations. Frac equipment was demobilizing off pad. The location meets the requirements for preliminary restorations within the Ohio Revised Code 1509.072 (A).
0582621975 2018-06-28 Casing String Witnessed No Violations The inspection was conducted as casing string inspection. The temperature is 82 degrees Fahrenheit. Ascent is setting 30-inch conductor pipe in 36-inch open hole. The tin horn measures 10 foot across and 13 feet deep. The 30-inch conductor inside diameter is 29.25 inch and weighs 118.76 lbs. /ft. The rig ran the conductor pipe to 100-foot-deep and had a shoe of 10 foot on the bottom. O-tex cementing services had mixed 67.3 barrels of slurry and displaced down with 75 barrels of fresh water. The slurry was mixed with 50 percent excess. There was good circulation with 10 barrels of slurry returns.

During my inspection the well was equipped with a production tree style wellhead with open valves. Gauges present read 0 pounds per square inch on the 13.375-inch diameter surface casing, 0 pounds per square inch on the 9.625-inch diameter intermediate casing, 3000 pounds per square inch on the 5.5-inch diameter production casing, 1100 pounds per square inch on the production tubing. The production equipment on location consisted on seven sand traps, seven gas production units, three horizontal heater treaters, four compressors, six 400-barrel tanks, five 500-barrel tanks, two vapor recovery towers, and one combustor.
0912294923 2018-09-25 Drill / Deepen / Reopen No Violations The purpose of my inspection was a status check on drilling operations. Accompanied by ODNR Kenn Frye, during our inspection we completed a walk around with Ascent Resources Superintendent James McGee. Patterson 577 was tripping out of hole to pick up the lateral assembly after landing the curve and drilling to a depth of 8110 feet. The kick off point was at 6742 feet. The anti-collision plan was evaluated.

1537352317 2018-09-19 Casing String Witnessed No Violations 13.35-inch diameter surface casing was ran to 1225 feet within the 17.5 inch diameter borehole. During my inspection I witnessed C&J Services O-Tex Cementing rig up on location. A safety meeting was held with all personnel on location. Following the safety meeting O-Tex pumped 50 barrels of gel, 10 barrels of fresh water, 1265 sacks (271 barrels) of class A cement, and 180 barrels for displacement. The cement was weighed in pressurized mud scales confirming the weight of 15.6 pounds per gallon. Cement returns were established.
1611361793 2019-01-18 Completion Testing No Violations The purpose of my inspection was a status check on drilling out frac plugs. During my inspection Fortis Energy Services had a derrick service rig on location. Frac plugs have been drilled out and 6.5 pound per foot 2.875-inch diameter production tubing had been installed.
