0007103977 2019-02-19 Completion Testing No Violations The purpose of my inspection was a status check. The five wells on location were all shut in waiting for the new pipeline to arrive. During my inspection the well had not been flowed back after completion. The well was equipped with a production tree style wellhead. All valves were chained and locked in the closed position. The valves leading to annular pressure gauges were in the closed position. The production equipment consisted of six 400-barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, and one combustor. The identification was posted at the well, tank battery, and entry.
0764097180 2019-04-03 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead. The production valves were in the open position. Select Energy Services were on location managing flowback operations on the five wells on the pad. I spoke with the Ascent Company man on location and he stated that the annular pressure was being monitored. The production equipment consisted of six 400-barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, and one combustor. The identification was posted at the well, tank battery, and entry.
0881288474 2018-10-30 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a production tree style wellhead. Production valves were in the closed position with chains and locks on all gate valves. The production equipment consisted of six 400-barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, and one combustor. The area effected by the pad construction has been restored with vegetation coverage on the lease road and pad perimeters. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry. Roustabouts were on location assembling production lines for production equipment.
1521476448 2018-03-11 Casing String Witnessed No Violations On the day of my inspection accompanied by ODNR Inspector Ryan Armstrong, I witnessed that Express Energy Services had drilled a 36 inch diameter hole to the depth of 100 feet and ran 100 feet of 30 inch 118.65 pound per foot conductor pipe. Express performed a 400 pounds per square inch pressure test, gained circulation, pumped 8 barrels of water and cemented pipe with 315 sacks Class A cement with 2% calcium chloride and .25 pound per sack of poly-flake with a weight of 15.6 pounds per gallon and a yield of 1.18 cubic foot per sack. Cement weight was verified by mud scales. Cement was displaced to 90 feet with 74.8 barrels of fresh water circulating 20 barrels to surface. Mix water was tested revealing chlorides of 68 parts per million, pH 7 and 58 degrees.
1526036154 2018-05-10 Drill / Deepen / Reopen No Violations During my inspection Patterson drilling rig 577 was preparing to pick up the bottom hole assembly with the 17.5 inch diameter bit to spud the surface section of the wellbore. The 13.375 inch diameter surface casing was being hauled on to location and placed on the pipe racks. I spoke with the Ascent company man and he stated that the cement job for the 13.375 inch casing will possibly be Friday evening on 5/11/18.

During my inspection Deep Well Services was rigging down and preparing to move off location. The frac plugs have not yet been drilled out and the well was equipped with a Frac valve.
