0302763178 2019-04-25 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present with each valve identified with casing information. The 13.375-inch diameter casing had 0 pounds per square inch and the 9.625-inch diameter casing had 1250 pounds per square inch. The production equipment consisted of eight 400-barrel tanks, two vapor recovery towers, five sand traps, five gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.
1040540495 2017-09-12 Casing String Witnessed No Violations On the day of my inspection I witnessed that Falcon Drilling Rig #26 had drilled a 17.5 inch diameter hole to the depth of 1317 feet and Wyoming casing crew ran 1307.5 feet of 13.375 inch 61 pounds per foot J-55 casing 3.5 feet above KB. Falcon Drilling Rig #23 washed down the last 9 joints of casing pumping a total of 230 barrels of fresh water. O-Tex Pumping LLC was onsite pumped 5 barrels of fresh water and performed a 2000 pound per square inch pressure test. O-Tex pumped 35 barrels of gel and cemented casing with 1385 sacks of Class H cement with 1.5% Calcium Chloride, .2% X--flake weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement weight was verified by pressurized mud scales and was displaced with 192.2 barrels of fresh water. Had good cement returns of 52 barrels, plug was landed at 600 pounds per square inch, pressured to 1130 pounds per square inch for 5 minutes and the floats held with 1 barrel back Mix water was tested revealing chlorides of <32 parts per million, pH 7 and 61 degrees.
1147924901 2017-09-11 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Falcon Drilling Rig #26 was rigged up and hooking up their flowline in preparation of commencement of drilling the 17.5 inch diameter surface borehole. I met with Ascent Consultant Randy Shepherd and reviewed the drilling proposal. Falcon Drilling Rig #26 will drill the 17.5 inch diameter wellbore to 1305 feet and set 13.375 inch diameter 61 pounds per foot casing with cementing to follow.
1172147403 2018-07-06 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of eight 400 barrel tanks, two vapor recovery towers, five sand traps, five gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.
1231329329 2017-09-01 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Precision Rathole had drilled a 36 inch diameter hole to the depth of 100 feet and was preparing to 30 inch diameter 118.65 pound per foot conductor pipe to total depth with cementing scheduled for tomorrow.

I witnessed the 30 inch diameter conductor pipe had been set at 100 feet and cemented on Saturday and the mouse hole has been drilled. Precision Rathole was onsite and preparing to run and set the 16 inch diameter pipe.

1508765937 2017-10-23 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #577 was onsite, rigging up, preparing to raise the derrick and plans to re-spud the well tomorrow. I met with Ascent Consultants Cecil Lovell and Ron Tomlinson and reviewed drilling proposal, testing, logging and cementing requirements for the 34059245080000 GIL NE LND GR 3 and the 34059245100000 GIL NE LND GR 7. A walk around was completed revealing secondary containment is in place along with viewing the derrick being raised to the drilling position.
1509977199 2017-11-06 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #577 had reached total depth on their lateral at 18373 feet and set 5.5 inch diameter 23 pound per foot P-110 casing at 18370 feet. O-Tex Pumping LLC, had just landed the plug at 07:30 at 3250 pounds per square and pressured up to 3780 pounds per square inch for five minutes with the floats holding. Casing was cemented with a lead 680 sacks of 50/50 H/Poz cement and a tail of 2450 sacks of Class H cement with full returns throughout the job. O-Tex was rigging down with plans to walk the rig to the 3H later today. I completed walk around with Ascent Consultant Burl Miller.
1523552049 2018-04-12 Completion Testing No Violations The purpose of my inspection was to follow up on complaint number 5817. During my inspection Halliburton and Allied Horizontal Wireline Services were on location. Halliburton was completing hydraulic stimulation operations on stage 54 out of 91 on the Gil 1H 3405924507 and stage 45 out of 61 on the Gil 3H 3405924508. I discussed operations with Ascent Company man Matt Bollinger. Mr. Bollinger stated that operations have been moving forward without issue.
