0097222182 2017-12-08 Production Wells No Violations On the day of my inspection I witnessed the well was equipped with a production tree with valves in the closed position and the valves locked. The production equipment included a GP Unit and a sand trap. Also present 4-500 barrel tanks within containment, a horizontal heated separator, a vapor recovery tower, 3-compressor units, a vertical vent scrubber and a gas combustor all shared with 34059244900000 Troyer LND GR 5H. SEI is onsite installing a cage over the production tree. Express Energy is also onsite preparing to set cellars and then drill conductors on the 34059244920000 Troyer LND GR 1H, 34059244930000 Troyer LND GR 3H, 34059244660000 Troyer LND GR 9H and 34059244740000 Troyer LND GR 11H.

0170050912 2018-07-10 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of seven 500 barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vent scrubber, and one combustor. The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry.
0415058095 2017-03-07 Completion Testing No Violations On the day of my inspection I met with Ascent Construction Supervisor Jordy Bernard and completed walk around revealing 4-500 barrel tank and a vapor recovery tower had just been set within containment. Also the gas production unit has been set along with excavation of the flowline ditch. Construction will be ongoing with a planned flowback date of April 8, 2017.

Gauges present read 2500 pounds per square inch on the tubing and 2600 pounds per square inch on the casing with annular pressure monitoring in place. The production equipment included a GP Unit and a sand trap. Also present 4-500 barrel tanks within containment, a horizontal heated separator, a vapor recovery tower, 3-compressor units, a vertical vent scrubber and a gas combustor all shared with 34059244650000 Troyer LND GR 7H.
0982440730 2016-12-27 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #84 was drilling their lateral on 10.7 pound per gallon mud at a depth of 10774 feet with a planned total depth of 16476 feet. I met with Ascent Consultant Russ White and discussed the 9.625 inch intermediate cement job as I was notified on 12/24/16 that they had lost returns prior to tripping out of the wellbore. Nomac Drilling Rig #84 had spotted two gel pills prior to tripping out and spotted a third gel pill after landing casing. They gained circulation and Schlumberger successfully cemented the casing with a 2-stage job circulating 46 barrel of cement to surface on the first stage and 43 barrels circulated to surface on the second stage. Ascent will be running a bond log upon completion of the lateral and will supply ODNR with a copy.

1482152670 2016-12-19 Casing String Witnessed No Violations I am on location today to cover the 13-3/8 surface casing cement job for inspector Doug Bennett. He will be covering the casing ticket for this job. I am just on location to witness the cement returns back to the surface. Schlumberger cementing used 1375 sacks D9D1 at 15.6 ppg for the cement job. We had 55 barrels of cement return back to the surface. The cement did not drop back down hole.

1488385621 2017-02-28 Completion Testing No Violations Witnessed Cudd Energy services rig down and move off well. Keane Frac performed 12,000 pounds per square inch pressure test on Frac stack and pumped 50 barrel gel spacer to clear wellbore. Allied Wireline Horizontal services were on location and rigged up bridge plug, perforating guns, lubricator and rig up to wellhead. Keane performed pressure test and pumped guns down wellbore at 16 barrels a minute set bridge plug at 16109 feet and perforated the 5.5 inch diameter production casing with 30 shots from 16098 feet to 15978 feet. Accompanied by ODNR inspector Doug Bennett.

1831093715 2016-10-15 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Precision Rathole had drilled a 36 inch diameter hole to the depth of 100 feet and ran 100 feet of 30 inch 118.65 pound per foot conductor pipe. Allied Oil & Gas Service was on site gained circulation, pumped 20 barrels of water and cemented pipe with 335 sacks Class A cement with 2% calcium chloride and .25 pound per sack of poly-flake with a weight of 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement weight was verified by pressurized mud scales. Cement was displaced to 90 feet with 72 barrels of fresh water. Good returns of 30 barrels and cement stayed at the surface.
