I met with Ascent Consultant Alan Rice and completed walk around and reviewed the upcoming cement job.

During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of five 500 barrel tanks, one vapor recovery tower, three gas production units, one compressor, one vertical vent scrubber, and one combustor. The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry.
1515603314 2018-01-10 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Southeast Regional Manager Brent Bear I witnessed Patterson UTI Drilling Rig #584 was rigging up. We met with and completed walk around with Ascent Consultant Josh Delso.
1523375882 2018-04-09 Completion Testing No Violations The purpose of my inspection was a status check on completions. During my inspection Deep Well Service Snubbing unit 11 was currently tripping out of hole after completing drill out operations. The rig has completed drilling out the frac plugs on the entire pad and will be rigging down and moving off of location.
1523624481 2018-04-12 Completion Testing No Violations The purpose of my inspection was to follow up on complaint 5815. The pad was named Betts and consisted of 3 wells. During my inspection I witnessed that Deep Well snubbing unit 11 was rigged down and moved off of location, the production tubing has been ran, and the wells 3405924483 and 3405924484 are being prepared for flow back. The production equipment consisted of a sand trap, three gas production units, one vapor recovery tower, two compressors, one vent scrubber, one 400 barrel gas buster tank, and one combustor. There was not any smoke coming from the combustor during inspection.

2032443498 2016-10-05 Casing String Witnessed No Violations The purpose of my inspection today is to witness a 100 feet of a 30 inch diameter conductor being cemented into a 36 inch diameter hole. O-Tex, Precision Rathole, and Ascent are on location. O-Tex pumped 10 barrels of fresh water ahead of 64 barrels of Class A cement weighing 15.6 pounds per gallon. 69 barrels of water were used for displacement. Cement returns were established. During the drilling of this 36 inch conductor hole, no water was found.
