0022117796 2016-11-23 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #84 was drilling their lateral with an 8.75 inch diameter bit at 13196 feet on 11.7 pound per gallon mud and a projected total depth of 18200 feet. I received notification on 11/21/16 after landing the curve they started picking up gas between 4000 and 5000 units and flaring of the well was on and off. At the time of inspection they were seeing 5600 units with an intermittent 3 foot flare. I completed walk around with Ascent Consultant Russ White revealing proper containment was in place and the 5.5 inch 23 pound per foot production casing was being offloaded and staged.
0056341590 2017-02-24 Completion Testing No Violations On the day of my inspection accompanied by ODNR Inspector Doug Bennett, Deep Well Services Snubbing Unit # 1 was laying down 2.875 inch diameter drill out tubing with Nine Wireline Services preparing to trip inside the 5.5 inch diameter casing and set a tubing packer at 7100 feet and 2.875 inch diameter production tubing to follow. We completed walk around with Ascent Consultant Joe Hartley.

Gauges present read 2450 pounds per square inch on the tubing and 2500 pounds per square inch on the casing with annular pressure monitoring in place. The production equipment included a GP Unit, a sand trap, 4-500 barrel tanks within containment, a vapor recovery tower, 2-compressor units, a vertical vent scrubber and a gas combustor.

0664817134 2016-10-04 Casing String Witnessed No Violations The purpose of my inspection today is to witness a 100 feet of a 30 inch diameter conductor being cemented into a 36 inch diameter hole. 10 barrels of fresh water was pumped ahead of 77 barrels of Class A cement weighing 15.6 pounds per gallon. Cement returns were established. During the drilling of this 36 inch conductor hole, no water was found.

During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of five 500 barrel tanks, one vapor recovery tower, three gas production units, one compressor, one vertical vent scrubber, and one combustor. The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry.
1031462640 2016-11-08 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Nomac Drilling Rig #84 rigging up with the sub structure in place and the back yard being set up. I met with Ascent Consultants Vernon Gallo and Josh Delso and reviewed the casing program. Mr. Delso stated that the derrick to arrive later this week and re-spudding of the well is scheduled for this weekend.

1152541872 2017-02-15 Completion Testing No Violations On the day of my inspection I witnessed Allied Horizontal set a frac plug at 13422 feet then perforate 5.5 inch diameter production casing with 30 shots from 13411 feet to 13270 feet. Keane Frac was onsite and I then witnessed frac stage 19 of 46 with a breakdown pressure of 8084 pounds per square inch, an average treating pressure of 9430 pounds per square inch, an average rate of 84.2 barrels per minute and a total of 251180 pounds of proppant pumped. I completed walk around with Consultant Kevin Hudson revealing proper secondary containment was in place.

1373093049 2016-11-13 Casing String Witnessed No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #84 had drilled a 17.5 inch diameter hole to the depth of 1353 feet and Wyoming casing crew ran 1357 feet of 13.375 inch 61 pounds per foot J-55 casing 4 feet above KB. Nomac Drilling Rig #84 washed down the last 2 joints of casing. Schlumberger was on site and gained circulation with 5 barrels of fresh water, 30 barrels of gel, 5 barrels of fresh water and cemented casing with 1500 sacks of Class A cement weighing 15.6 pounds per gallon and a yield of 1.18 cubic foot per sack with 0.2% Antifoam, 0.1% Dispersant, 0.5% FLAC and .1% Lost circulation material. Cement weight was verified by mud scales and displaced with 193 barrels of fresh water. Had good cement returns of 83 barrels, plug was landed at 477 pounds per square inch, pressured to 1030 pounds per square inch and the floats held with 1 barrel back. Schlumberger pressured to 1030 pounds per square inch for 5 minutes and the floats held with 1 barrel back. Mix water was tested revealing chlorides of <33 parts per million, pH 7 and 56 degrees.

2080702153 2018-04-12 Production Wells No Violations The purpose of my inspection was to follow up on complaint 5815. The pad was named Betts and consisted of 3 wells. During my inspection I witnessed that Deep Well snubbing unit 11 was rigged down and moved off of location, the production tubing has been ran, and the wells 3405924483 and 3405924484 are being prepared for flow back. The production equipment consisted of a sand trap, three gas production units, one vapor recovery tower, two compressors, one vent scrubber, one 400 barrel gas buster tank, and one combustor. There was not any smoke coming from the combustor during inspection.
2085244615 2017-03-07 Completion Testing No Violations On the day of my inspection I met with Ascent Construction Supervisor Jordy Bernard and completed walk around revealing 4-500 barrel tank and a vapor recovery tower had just been set within containment. Also the gas production unit has been set along with excavation of the flowline ditch. Construction will be ongoing with a planned flowback date of April 8, 2017.
