0191634064 2019-04-30 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with production valves in the open position. Annular pressure monitoring was present the 13.375-inch diameter intermediate casing had 300 pounds per square inch and the 9.625-inch diameter intermediate casing had 350 pounds per square inch. The production equipment consisted of four 500-barrel tanks, one high low separator, one gas compression skid, one vertical vent scrubber, and two combustors. Identification was present and production records were current.

1451744225 2016-01-02 Casing String Witnessed No Violations On the day of my inspection I witnessed that Hall Drilling Rig #5 had drilled a 17.5 inch diameter hole to the depth of 1497 feet and ran 1435 feet of 13.375 inch 54.5#/ft. P-110 intermediate casing. Universal Well Services was on site and gained circulation and pumped 25bbls of gel, 5bbls of fresh water and cemented casing with a lead of 615 sacks of Gas Block cement weighing 14.5ppg with 2% CaCl, .3% CR3 and .25#/sk. of cello flake followed by 490 sacks of Gas Block cement weighing 15.7ppg with 2% CaCl, .3% CR3 and .25#/sk. of cello flake. Cement weight was verified by pressurized mud scales and displaced with 215bls of fresh water. Had good cement returns of 34bbls, plug was landed at 627psi, pressured to 1132psi for 5 minutes and the floats held. Universal left pressure at 627psi and shut-in until wellhead is installed tonight.

Hall Drilling Rig # 5 was rigging down and being released.
1456847850 2016-03-01 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Precision Drilling Rig #110 had completed the BOP tests, tagged up at 4354 feet and were currently drilling a 12.25 inch diameter hole at a depth of 5104 feet with a planned td of 6300 feet.
1457353252 2016-03-07 Drill / Deepen / Reopen No Violations On the day of my inspection Precision Drilling Rig #110 had drilled a 12.25 inch diameter hole to the depth of 6457 feet, ran and set 6407 feet of 9.625 inch 40#/ft. casing with a DV tool being set at 3933 feet. Universal Well Services, Inc. was on site and completed a 2 stage cement job on the casing. The first stage was cemented with a lead of 357 sacks of Unifill light cement with .25% CR-3 and .25#/sk. Celloflake weighing 12.7ppg and a yield of 1.84cuft/sk. Lead was followed by a tail of 17 sacks of Type 1 cement with 2% gel, .75% CFL-117, .3%CR3 and .25#/sk. Celloflake weighing 15.0ppg and a yield of 1.31cuft/sk. Cement was displaced with 286bbls of 10ppg mud, 10bbls of fresh water and 186bbls of 10ppg mud. Plug was landed at 500psi and pressured up to 1000psi with the floats holding. Universal dropped the free fall plug and waited 20 minutes before pressuring up to 490psi opening the DV tool. Universal pumped a 270bbl bottoms up circulating 35bbls of cement to surface from the first stage. Precision Drilling Rig #110 circulated on casing for 4 hours. Universal Well Services then pumped 10bbls of fresh water followed by 873 sacks of Type 1 cement with 6% gel, 10% Salt, 1% CFL-117 and .25% Foam Chek-
1458564670 2016-03-21 Drill / Deepen / Reopen No Violations On the day of my inspection Precision Drilling Rig #110 completed cementing of their production casing on March 19th and was rigged down with Deep Well Services preparing for rig move.
1461682620 2016-04-26 Completion Testing No Violations On the day of my inspection I witnessed stage 4 of 40 had been completed prior to arrival by Halliburton Services with FTS Wireline Services rigging up the next perforating gun. FTS tripped in the wellbore at an average rate of 14.3bbls/min and set stage 5 plug at 16497 feet and began tripping out of the successfully perforating the 5.5 inch production casing with 40 shots from 16488 feet to 16302 feet.

Met with Consultant Jim Lake and Drilling Superintendent Kevin Rothenbuhler reviewing remaining plugs to be drilled out, plans for production equipment installation and bringing the well online.
