0124787059 2018-01-03 Drill / Deepen / Reopen No Violations On the day of my inspection at -10 degrees, I witnessed after 3 trips from the wellbore due to motor failures Precision Drilling Rig #110 was drilling ahead in the intermediate section with a 12.25 inch diameter bit at 3429 feet with a planned total depth of 7150 feet. I completed walk around revealing proper secondary containment was in place and 9.625 inch diameter 40 pound per foot casing has been offloaded for upcoming job. KLX Energy Services was onsite preparing to make up the shoe track.

1352095471 2018-01-09 Drill / Deepen / Reopen No Violations On the day of my inspection following up from a call at 00:10 where Precision Drilling Rig 110 stated that during the 2nd stage of the 9.625 inch diameter casing cement job returns were lost at 200 barrels into displacement. Total displacement of the 2nd stage was 281.7 barrels. I met with Ascent Resources Consultant Heath Satterfield and discussed the job. Mr. Satterfield stated that a cement bond log will be performed prior to reaching total depth and will be forwarded.
1514558786 2017-12-28 Casing String Witnessed No Violations The purpose of my inspection was to witness 1255 feet of 13.375 inch diameter surface casing being cemented into a 17.5 inch diameter borehole. I witnessed Wyoming casing crew run the entire string in hole. A safety meeting was held with all personnel on location including Schlumberger, Precision Drilling, and Ascent Company Men. Water and gel returns will be taken to the half rounds and when the returns change over to cement the water trucks will vacuum out the cement returns out of the cellar. Following the safety meeting Schlumberger pressure tested their lines and pumped 25 barrels gel spacer, 5 barrels of fresh water, and 288 barrels of class H cement at 15.6 pounds per gallon. A cement sampled was pulled and weighed in pressurized scales confirming the weight of 15.6 pounds per gallon. The top plug was dropped and 160 barrels of fresh water were pumped for displacement. At 100 barrels within displacement cement retuned to surface and at 160 barrels within displacement the water trucks on location could no longer vacuum up the cement returns. At that point the Ascent Superintendent shut the job down due to being overcome with cement returns and having no place for them to go. Ascent plans to wait on the cement to reach 500 pounds per square inch compressive strength and then pressure up on the casing with 500 pounds per square inch and hold that pressure for 30 minutes.
1516370075 2018-01-19 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Precision Drilling Rig #110 had reached total depth in their lateral at 16975 feet and Wyoming Casing Crew had just landed 5.5 inch diameter 23 pound per foot P-110 casing at 16975 feet and was rigging down. Schlumberger was onsite rigging up to cement the casing. I met with Ascent Consultant John Wilson and completed a walk around along with reviewing the cement pump schedule for the upcoming job.
1588990744 2017-12-20 Drill / Deepen / Reopen No Violations On the day of my inspection I met with Ascent Resources Consultant Steve Derment and reviewed the drilling proposal. Precision Drilling Rig #110 was onsite rigging up with plans to commence drilling on Saturday.
1664531945 2017-12-11 Casing String Witnessed No Violations On the day of my inspection I witnessed that Express Energy Services had drilled a 36 inch diameter hole to the depth of 100 feet and ran 100 feet of 30 inch 118.65 pound per foot conductor pipe. Express Energy Services performed a 300 pounds per square inch pressure test, gained circulation, pumped 4 barrels of water and cemented pipe with 275 sacks Class A cement with 2% calcium chloride and .25 pound per sack of poly-flake with a weight of 15.6 pounds per gallon and a yield of 1.18 cubic foot per sack. Cement weight was verified by mud scales. Cement was displaced to 85 feet with 70 barrels of fresh water. Mix water was tested revealing chlorides of <32 parts per million, pH 7 and 63 degrees. Cement fell 10 feet from surface and will be checked and topped off tomorrow.
2023871759 2017-12-13 Casing String Witnessed No Violations On the day of my inspection I witnessed that the top of cement was at 35 feet from surface. Express Energy Services was onsite and topped off conductor with 64 sacks of 9 sack grout and cement stayed at the surface.
2104553953 2018-07-10 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of seven 500 barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vent scrubber, and one combustor. The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry.
