0844625905 2018-02-09 Production Wells No Violations On the day of my inspection I witnessed 2.875 inch diameter tubing has been tripped out of the well and a master valve installed with all valves in the closed position. A meeting with Ascent Consultant Shawn Fox revealed that two downhole plugs were set inside the 5.5 inch diameter casing at 6502 feet and 6412 feet in preparation for Patterson UTI Drilling Rig #584 drilling the 34059244680000 Shipman WLS GR 4H and 34059244670000 Shipman WLS GR 2H.
1024239112 2017-03-03 Drill / Deepen / Reopen No Violations On the day of my inspection following up on Complaint ID 5543 and accompanied by ODNR Southeast Regional Manager Brent Bear, ODNR Geologist Jake Glascock and Soil and Water Wildlife Forestry Specialist Levi Arnold. We completed walk around of the wellsite revealing the extra counter measures that have been put in place to prevent any silt reaching the landowners pond. The fuel tank that was in question has also been removed.
1028707333 2017-03-02 Drill / Deepen / Reopen No Violations On the day of my inspection following up on Complaint ID 5543 and after 2 inches of rain which fell on 3/1/16, I performed field tests on the south sump revealing a pH of 7 and chloride level of 43 parts per million (ppm). Field tests were performed on the north sump revealing a pH of 7 and chloride level of 43 ppm. I also performed field tests on the cellar of the 6H revealing a pH of 7 and chloride level of 43 ppm. Silt sock has been installed in the rock check of the south sump outfall with all water being released on the north outfall. Silt sock has also been placed at the southwest of the access road near the rock check leading to the pond. No substantial erosion is taking place and the fuel tank has not been fueled and is scheduled to be moved to a more suitable location.
1047559087 2017-03-18 Completion Testing No Violations I received a call from Ascent Government Relations Manager Amanda Finn at 22:21 on 3/17/17. Ms. Finn stated that a truck hauling sand had flipped over along the access road entering the Shipman Pad. She also stated that there were no severe injuries and that measures were put in place to prevent any contaminates to reach the land owners pond. The sand would be offloaded, the truck would be righted and testing would be done on the affected area. Upon arrival on 3/18/17 I witnessed the Apex Energy Services was completing the removal of the sand and Lloyds Wrecker Service was onsite with two trucks preparing to right the truck. Walking around the truck revealed absorbent socks had been placed west of the truck and containment was placed under the fuel tank capturing the small drip from the tank. Truck was righted with minimal impact to the area and WSW Environmental took soil samples to their laboratory for evaluation. Extra absorbent socks were laid out for countermeasures around impacted area while waiting on the lab results.
1149754750 2017-01-04 Drill / Deepen / Reopen No Violations On the day of inspection accompanied by ODNR Senior Health Physicist Paul Carder, I witnessed that Nomac Drilling Rig #80 was moving in and rigging up with plans to commence drilling on Friday. We met with Ascent Consultant Colden Parker and completed walk around.
1269446329 2016-12-10 Casing String Witnessed No Violations On the day of my inspection I witnessed that Falcon Drilling Rig #26 had drilled a 17.5 inch diameter hole to the depth of 1180 feet and Wyoming casing crew ran 1155 feet of 13.375 inch 61 pounds per foot J-55 casing 4 feet above KB. Falcon Drilling Rig #26 washed down the last 4 joints of casing. Allied Oil and Gas Services was on site and gained circulation with 2 barrels of fresh water, 50 barrels of gel, 5 barrels of fresh water and cemented casing with 1280 sacks of Class H cement weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack with 2% Calcium Chloride and .25 pound per sack poly-
1481033005 2016-12-06 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Apex Energy Services installing rig containment in preparation for the arrival of Falcon Drilling Rig #26 to drill the surface section of the well.
1484141814 2017-01-11 Drill / Deepen / Reopen No Violations On the day of inspection I witnessed that Nomac Drilling Rig #80 had reached a total depth of 6293 feet with 12.25 inch diameter bit and Wyoming Casing Crew had ran 9.625 inch diameter 40 pounds per foot casing to 6286 feet. Schlumberger had completed the first stage of the job and opened the DV tool which was set at 3385 feet. Nomac Drilling Rig #80 then circulated through the tool and 73 barrels of the 80 barrels of Mudpush that was pumped on the first stage was pumped to surface. Safety meeting was held as Schlumberger prepared to begin the second stage. Due to a scheduled pre-site I had to leave the job. At 12:20, I received call from Ascent Consultant Josh Delso stating they did not get cement returns to surface on the second stage and the top is calculated at 650 feet. I stated that Ascent would have to prove where top of cement was at with the 13.375 inch diameter surface casing being set at 1155 feet so the top of cement has to be 200 feet inside. Mr. Delso stated that bond log would be run once total depth is reached. I stated that if the bond log is not satisfactory that Ascent would have to submit a plan for remediation to be in compliance with OAC 1501:9-1-08 (6) (c) and (d)
1484569324 2017-01-15 Drill / Deepen / Reopen No Violations Per the conversation held on 1/12/17 on Nomac Drilling Rig #84 with Ascent Consultants Josh Delso and Burl Miller, concerning cement not being circulated to surface on the second stage of the intermediate casing cement job. It was reviewed that the surface casing was set at 1155 feet and top of the Berea was at 1390 feet. The top of cement (TOC) should at a minimum be at 890 feet to meet both sections of OAC 1501:9-1--1-08 (N).
1503563795 2015-05-10 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Precision Rathole had drilled a 36 inch diameter hole to the depth of 100 feet and ran 100 feet of 30 inch 118.65#/ft. conductor pipe. Allied Oil & Gas Service was on site gained circulation, pumped 20bbls of gel and cemented pipe with 310 sacks Class A cement with 2% CaCl and .25#/sk. of poly-flake with a weight of 15.6ppg and a yield of 1.2cuft/sk. Cement weight was verified by pressurized mud scales. Cement was displaced to 90 feet with 75bbls of fresh water and had good cement returns of 15bbls to surface.
1508849161 2017-10-24 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a production tree and the valves were in the open position. The production equipment included a sand trap, a GP Unit, a heater treater, a vapor recovery tower, a compressor unit, a disconnected compressor unit, a heater treater, a vertical vent scrubber, a combustor unit and a total of 4-500 barrel tanks all within containment. Gauges present read 285 pounds per square inch on the tubing and 500 pounds per square inch on the casing with annular pressure monitoring in place.
1529001943 2018-06-14 Completion Testing No Violations On site inspection accompanied by ODNR North Region Emergency Response Coordinator Andrew Adgate. On this day it was observed that the drilling and completion process has been completed, the production tree well head assembles have been installed and crews are on site installing the production equipment. A production fluid facility has been erected with 5 steel construction 400 barrel production tanks present inside a polymer lined steel construction dike area. There are a total of 3 wells on the pad present that have been completed.

I completed walk around with Ascent Consultant David Rapattoni revealing proper secondary containment was in place.

I placed call to Ascent Resources EHS Emergency Response Coordinator Rich Weber. Mr. Weber stated that areas were marked yesterday for additional erosion and sediment controls which should be installed today. Mr. Weber also stated that the fuel tank will be moved to a more suitable location. At 15:37 I received a call from OEPA Scott Foster who stated they had received a complaint from the landowner on silt entering his pond and we discussed the pad and the extra measures that were being put into place.

Mr. Shipman alleged that silt was being discharged from the pad and entering his pond. Walk around was completed revealing sump drain valves were in the shut position with no water being discharged from the pad and no substantial erosion.
