Silt sock has also been placed at the southwest of the access road near the rock check leading to the pond. No substantial erosion is taking place and the fuel tank has not been fueled and is scheduled to be moved to a more suitable location. |

Ms. Finn stated that a truck hauling sand had flipped over along the access road entering the Shipman Pad. She also stated that there were no severe injuries and that measures were put in place to prevent any contaminates to reach the land owners pond. The sand would be offloaded, the truck would be righted and testing would be done on the affected area. Upon arrival on 3/18/17 I witnessed the Apex Energy Services was completing the removal of the sand and Lloyds Wrecker Service was onsite with two trucks preparing to right the truck. Walking around the truck revealed absorbent socks had been placed west of the truck and containment was placed under the fuel tank capturing the small drip from the tank. Truck was righted with minimal impact to the area and WSW Environmental took soil samples to their laboratory for evaluation. Extra absorbent socks were laid out for countermeasures around impacted area while waiting on the lab results. |

Nomac Drilling Rig #80 then circulated through the tool and 73 barrels of the 80 barrels of Mudpush that was pumped on the first stage was pumped to surface. Safety meeting was held as Schlumberger prepared to begin the second stage. Due to a scheduled pre-site I had to leave the job. At 12:20, I received call from Ascent Consultant Josh Delso stating they did not get cement returns to surface on the second stage and the top is calculated at 650 feet. I stated that Ascent would have to prove where top of cement was at with the 13.375 inch diameter surface casing being set at 1155 feet so the top of cement has to be 200 feet inside. Mr. Delso stated that bond log would be run once total depth is reached. I stated that if the bond log is not satisfactory that Ascent would have to submit a plan for remediation to be in compliance with OAC 1501:9-1-08 (6) (c) and (d) |
