0141145418 2016-04-19 Production Wells No Violations On the day of my inspection I received a call from Ascent Resources EHS Supervisor stating there is a trail of fluid leaving the Eggleston well pad approximately 200 yards long, approximately 15-20gallons and that is was unsure what the product was. Upon arrival I witnessed that a release of condensate 18 inches wide by 700 feet beginning at 40.037908 -81.417536 and ending at 40.073887 -81.420024 of condensate was present. The release began at the entrance to the well pad and proceeded west on Easton Road. I met with Ascent Resources EHS Specialist Completion/Production Safety Joe Steele and discussed incident. Mr. Steele stated that SPSI was in route from Canton to begin clean up and that the release came from one of three trucks that left the location last night.
0276188989 2017-06-29 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a production tree and the valves were in the open position. The production equipment included a sand trap, a GP Unit, a line heater, a vapor recovery system, 1- NGL tank, a compressor unit, a heater treater, a stabilizer unit, a vertical vent scrubber, a combustor unit and a total of 6-500 barrel tanks all within containment and shared with the 4H. Gauges present read 180 pounds per square inch on the tubing and 480 pounds per square inch on the casing with annular pressure monitoring in place.
0490097560 2015-05-31 Preliminary Restoration No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #80 had drilled an 8.5 inch diameter lateral wellbore to the depth of 17430 feet and O-Tex cemented 17430 feet of 5.5 inch 23#/ft. casing. Jay Pappas was on site power washing the rig and cleaning the pits in preparation for rigging down and moving to the Pol pad in Belmont County. Inspection revealed the site passed preliminary restoration according to 1509.072A.
0578927239 2015-05-20 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Austin Gutridge, I witnessed that Nomac Drilling Rig #80 had drilled a 17.5 inch diameter hole to the depth of 1215 feet and Wyoming Casing Crew was onsite and ran 1215 feet of 13.375 inch 54.5#/ft. J-55 Surface Casing. O-Tex Pumping LLC was on site and gained circulation and pumped 50bbls of gel, 10bbls of fresh water and cemented casing with 1220 sacks of class H cement 15.6ppg, 1.2cuft/sk. with 2%CaCl, 0.2% X-

Precision had drill a 20 inch diameter mouse hole to the depth of 70 feet and ran 100 feet of 16 inch pipe with cementing of mouse hole later today.
1432645798 2015-05-26 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #80 was drilling an 8.5 inch diameter lateral hole at a the depth of 8244 feet on 11.4# SOBM with a planned td of 17450 feet. Cementing of 9.625 inch casing was completed on 5/24/15 with CBL to run after the well is drilled to total depth.
1442499885 2015-09-17 Completion Testing No Violations On the day of my inspection I witnessed Baker Hughes was on site fracing stage 21 of 60 with a breakdown pressure of 7043psi, average treating pressure of 8774psi, average rate of 85.7bbls/min with 252547lbs of propant pumped, initial shut-in pressure at 5048psi and 5 minute shut-in pressure at 4118psi. Completed walk around with Consultant Don Barker revealing no violations.

There remains a light dusting of Gator Sorb in the east bound lane of Easton Road with the remaining portion of the affected road clear. I met with Dillon Transport Safety Coordinator Joey Anderson who is trying to determine the events leading to the release were related to one of their drivers.
