The production equipment included a sand trap, a GP Unit, a line heater, a vapor recovery system, 1- NGL tank, a compressor unit, a heater treater, a stabilizer unit, a vertical vent scrubber, a combustor unit and a total of 6-500 barrel tanks all within containment and shared with the 2H. Gauges present read 750 pounds per square inch on the tubing and 950 pounds per square inch on the casing with annular pressure monitoring in place.

0770569616 2015-02-17 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Precision Rathole had drilled a 36 inch diameter hole to the depth of 100 feet and ran 100 feet of 30 inch conductor pipe with Universal Services cementing the conductor on Saturday. Precision had drill a 20 inch diameter mouse hole to the depth of 70 feet and ran 100 feet of 16 inch pipe with cementing of mouse hole later today.

1166108047 2015-08-13 Completion Testing No Violations On the day of my inspection I witnessed Producers Service Corp. was on site and treated the well with 1000 gallons of 15% HCL with 3 gallons of biocide and 3 gallons of corrosion inhibitor. Predator Pressure Control was on site and performed a pressure test on the lubricator at 5000psi, 10,000psi and 13,500psi with the test passing. Nine Wireline Division tripped in the well with PSC pumping behind beginning at the kick off point of 6800 feet 3bbls/min, 30 degrees in the curve at 7151 feet 6bbls/min, 60 degrees in the curve at 7530 feet 9bbls/min and 90 degrees in the curve at 8480 feet 15bbls/min. PSC maximum flow was 16bbls/min at 8501psi and a total of 379bbls pumped. NWD successfully set a plug at 15448 feet and pulled up and PPC performed a 30 minute pressure test at 13,000psi. NWD perforated 5.5 inch casing with 30 total shots from 15,432 feet to 15,312 feet.

Completed walk around with Consultant Dave Norris revealing location clean-up was nearly completed and no violations.

Annular pressure monitoring was present and evaluated. The production equipment consisted of two sand traps, two gas production units, one line heater, one heater treater, one vapor recovery tower, and six 500 barrel tanks.

1502170430 2018-03-26 Production Wells No Violations On the day of my inspection accompanied by ODNR Inspector Jeff Adams following up on a phone call received 3/24/18 at 14:24 from Ascent Government Relations Manager Amanda Finn. Ms. Finn stated at 23:25 while the lease operator was unloading the well to the gas buster tank the operator saw the level rising in the tank and as he went over to shut-in the sand trap the tank ignited. Both wells were immediately secured by hitting the emergency shut down (ESD) button stopping the source of the fire. Supervision was contacted immediately along with 911 for the fire department. The fire was extinguished by the local fire department with foam and the pad was secured. Upon arrival at 10:45 on 3/26/18, I witnessed both wells were shut-in and secure. We met with Ascent Production Superintendent Adam Pence and reviewed the incident and remedial action to take place. The damage onsite was contained to the northwest corner of the pad with no product being released off pad and minimal undetermined amount onsite. The equipment damaged included the gas buster which prior to the fire was in containment, an empty methanol tank, various wiring and a computer monitor. No impact to the pad or off pad was found. Sun Pro Services had been dispatched to the location and as a precaution sprayed Micro Blaze agent around the impacted area Mr. Pence stated that once he is able to connect to the system they may be able to determine a root cause. Mr. Pence also stated that the configuration will change with the gas buster being relocated to the southeast corner.
